HIV患者中洛匹那韦/利托那韦对伏立康唑谷浓度的影响分析

摘　　要：目的考察我院HIV患者应用洛匹那韦/利托那韦(400 mg/100 mg,bid)对伏立康唑谷浓度的影响,为临床合理联用两药提供参考。方法回顾性分析2017年~2019年我院伏立康唑和洛匹那韦/利托那韦联用(联用组)及单用伏立康唑(单用组)的HIV患者伏立康唑血药谷浓度结果。结果74例患者共行128次伏立康唑药物浓度监测。128例次浓度监测中,有89例次伏立康唑浓度在1.0~5.5 mg·L^(-1),达标率为69.5%。联用组浓度比单用组浓度低约32%,而联用组的体重日剂量比单用组高15.7%(P<0.05)。联用组的达标率明显低于单用组(47.2%vs 85.3%,P<0.05)。结论HIV患者联用伏立康唑和洛匹那韦/利托那韦时,利托那韦(100 mg,bid)能明显降低伏立康唑浓度,需要密切监测伏立康唑浓度,并需及时调整给药剂量,保证临床疗效。OBJECTIVE To evaluate the effect of lopinavir/ritonavir(400 mg/100 mg,bid)on voriconazole trough concentration in HIV patients.METHODS Trough concentration of voriconazole in HIV patients who used lopinavir/ritonavir or not was analyzed respectively in our hospital from 2017 to 2019.RESULTS 128 times of voriconazole concentration were monitored in 74 HIV patients,69.5%(89 of 128)within the target range(1.0-5.5 mg·L^(-1)).The concentration in combination group was 32%lower than that in monotherapy group,and the voriconazole dose in combination group was 15.7%higher than that in monotherapy group.A lower standard reaching rate of combination group compared with monotherapy group(47.2%vs 85.3%,P<0.05).CONCLUSION Due to the significant effect of low dose ritonavir on voriconazole trough concentration,it is necessary to monitor voriconazole concentration closely and adjust voriconazole dosage in time to insure clinical efficacy.
