临床药师对1例胰岛素制剂过敏的糖尿病患者降糖方案的个体化设计

摘　　要：目的通过对1例胰岛素制剂过敏的糖尿病患者病例分析,探讨了其过敏形成原因和方案制定,旨在为今后临床治疗过程中发生类似的胰岛素制剂过敏事件,提供借鉴和参考。方法1例糖尿病患者使用胰岛素治疗过程中出现过敏,多次换用不同种类胰岛素但过敏症状未明显好转,临床药师通过查找过敏原因,分析过敏机制等手段,判断过敏原,制定了门冬胰岛素联合地特胰岛素降糖方案。结果方案调整后,患者的胰岛素过敏症状未再发生,最终血糖控制平稳出院。结论临床药师可发挥药学特长,协助医师制定个体化的给药方案,保障患者的用药安全。OBJECTIVE To analysis one case of diabetic patient with insulin preparation allergy,in order to provide reference for the occurrence of similar insulin preparation allergy events in the future clinical treatment.METHODS Different insulin preparations were tried out after the patient was allergic to one kind of insulin preparation.However,the allergic symptoms were not improved significantly.The clinical pharmacist determined the allergen by finding out the cause of allergy,analyzing the mechanism of allergy.A hypoglycemic program was developed for the combination of insulin aspart and insulin detemir.RESULTS After the program adjustment,the patient′s symptoms of insulin allergy did not occur again,and finally the patient was discharged with stable blood glucose control.CONCLUSION Clinical pharmacists can give full play to their strengths in pharmacy,assist physicians to develop personalized drug delivery plans,ensure the safety of patients.
