全自动摆药机在住院药房应用的差错分析及改进措施

摘　　要：目的分析全自动摆药机在我院住院药房应用中的差错,以预防全自动摆药机的差错发生,提高摆药准确性,提高药学服务水平。方法通过收集我院住院药房2020年1月~12月全自动摆药机差错记录共3172条,对发生差错的时间、药品、片型、类型进行统计分析。结果按月份分析,差错率主要集中在4~8月份;按易差错药品分析,差错率主要集中在美托洛尔缓释片,巴氯芬片及西替利嗪片等药品;按易差错药品的片型分析,差错率主要是半片型和长条形片剂;从差错类型分析,主要包括包药袋科室互窜、药品数量错误、药品窜包及药品碎片等类型。结论通过对差错原因的分析,制定相关的改进措施,以减少摆药差错的发生,提高摆药机工作效率及准确性,更好地为临床服务,使病人用药安全有效。OBJECTIVE To analyze the errors in the application of automatic dispensing machine in inpatient pharmacy of our hospital,so as to prevent the errors of automatic dispensing machine,improve the accuracy of dispensing and improve the level of pharmaceutical care.METHODS A total of 3172 error records of automatic dispensing machine in inpatient pharmacy of our hospital from January to December 2020 were collected,and the time,drugs,tablets and types of errors were statistically analyzed.RESULTS The results were analyzed by month,and the error rate was mainly concentrated in April to August.According to the analysis of error-prone drugs,the error rate mainly concentrated in metoprolol sustained-release tablets,baclofen tablets and cetirizine tablets;According to the analysis of tablet type of error-prone drugs,the error rate is mainly half tablet and strip tablet;From the analysis of error types,it mainly includes the types of drug bag interchanging,drug quantity error,drug bag interchanging and drug fragments.CONCLUSION Through the analysis of the causes of the errors,the relevant improvement measures were formulated to reduce the occurrence of dispensing errors,improve the efficiency and accuracy of dispensing machine,better serve for clinical use,and make patients use drugs safely and effectively.
