某基层医院近五年临床药师参与临床会诊病例的分析

摘　　要：目的了解某基层医院近5年来临床药师参与会诊的现状以及会诊效果,改进工作模式供其他同行参考。方法对该院2016年1月至2020年12月临床药师参与的院内会诊病例进行统计分析。结果共纳入51例会诊,该院临床药学每年的会诊数量由2016年的0例增加至2020年的21例。会诊科室以外科居多(56.9%)其次是内科(41.2%),某些科室零会诊;会诊目的中以指导抗感染方案的制定或调整为主(92.2%);会诊建议被采纳的41例(80.4%),会诊72 h后治疗有效36例(87.8%)。结论基层医院临床药师会诊病例逐渐增多,作用逐步得到临床认可,但与大医院相比仍存在一定的距离。临床药师尚需不断提高专业技能,探索和改进工作和会诊的模式。OBJECTIVE To understand the present status and effect of clinical consultations involving pharmacist in a primary hospital in the past 5 years,and to improve consultation model as a reference for pharmacists.METHODS Clinical consultations pharmacist involved in the primary hospital were analyzed between January 2016 and December 2020.RESULTS A total of 51 clinical pharmacist consultations were included.The number of consultations gradually increased from 0 in 2016 to 21 in 2020.The majority consultations were initiated by surgery department(56.9%),followed by internal medicine department(41.2%),while certain departments had nerve requested pharmacists′suggestion.Most(92.2%)of the consultations were to guide the formulation or adjustment of anti-infection therapy.The acceptance rate of consultation was 80.4%(41/51)and the effective rate evaluated at 72 hours post consultation was 87.8%(36/41).CONCLUSION The role pharmacists played in clinical consultations has increased and been recognized by clinicians,but it in the primary hospital is far behind that in senior hospital.Pharmacists are needed to improve professional skills and explore a better mode of consultation.
