摘 要:目的:探究体素不相干运动(IVIM)成像在脑胶质瘤分级中的的应用价值。方法:回顾性分析48例经手术病理证实为脑胶质瘤患者的临床资料及术前影像资料,分为高级胶质瘤(HGG)组39例和低级别胶质瘤(LGG)组9例,得到相应ADC值、真实扩散系数(D)、假扩散系数(D*)及灌注分数(f)值。采用两独立样本t检验比较2组的IVIM参数差异。使用ROC曲线分析IVIM参数的诊断性能。结果:HGG组的瘤体区ADC值和D值较LGG组低(均P<0.05),LGG组的D*值明显低于HGG组(P<0.05),在Ⅱ、Ⅲ级别间仅D值差异有统计学意义(P<0.05),在Ⅱ、Ⅳ级别间ADC值、D值、D*值和f值差异均有统计学意义(均P<0.05),在Ⅲ、Ⅳ级别间ADC值、D值、f值差异均无统计学意义(均P>0.05)。ROC曲线显示,在区分HGG与LGG方面,瘤体区D值的AUC高于ADC值,且D值与ADC值密切相关(r=0.959,P<0.001)。结论:IVIM参数中D值能够有效鉴别HGG与LGG脑胶质瘤,在两者的分级中具有一定应用价值。Objective:To explore the value of IVIM in the grading of gliomas.Methods:The clinical data of 39 cases of high grade gliomas(HGG)and 9 cases of low grade gliomas(LGG)confirmed by pathology were analyzed retrospectively.The parameters of IVIM,including ADC,true diffusion coefficient(D),false diffusion coefficient(D*)and perfusion fraction(f)values were obtained.Two independent samples t-test was used to compare the differences of IVIM parameters between HGG and LGG group.ROC curve was used to analyze the diagnostic performance of IVIM parameters.Results:The ADC values and D values in HGG group were lower than those in LGG group(P<0.05),the D*value of LGG was significantly lower than that of HGG(P<0.05).Only the D value had significant difference betweenⅡandⅢgrades(P<0.05).There were significant differences in ADC,D,D*and f values betweenⅡandⅣgrades(all P<0.05).There were no statistical differences betweenⅢandⅣgrades(all P>0.05).In all gliomas,AUC of the D value in the tumor area was lower than that of ADC,and there was a close correlation between the two parameters(r=0.959,P<0.001).Conclusion:D value of IVIM can effectively differentiate HGG and LGG,and it has certain application value in glioma grading.
