摘　　要：目的:探讨定量磁化率成像(QSM)对脑微出血(CMBs)检出与鉴别诊断的价值。方法:回顾性分析SWI检出CMBs,且常规序列、DWI及QSM资料完整的患者40例,比较不同序列对CMBs的检出情况及病灶分布特点。结果:CMBs主要分布于皮质下区及基底节区,SWI及QSM可清晰显示CMBs形态学特征。QSM、常规序列及DWI序列在检出CMBs阳性方面,差异有统计学意义(χ^(2)=48.49,P<0.001);SWI、QSM、常规序列在不同脑区检出CMBs的数目差异有统计学意义(χ^(2)=43.40,P<0.001);SWI与QSM检出各脑区CMBs数目差异无统计学意义(χ^(2)=8.72,P=0.07)。结论:QSM对CMBs的识别和鉴别准确性较高,可作为SWI序列的补充,具有一定临床应用价值。Objective:To explore the diagnostic value of quantitative susceptibility mapping(QSM)in the detection and differentiation of cerebral microbleeds(CMBs).Methods:40 CMBs patients detected by magnetic sensitivity weighted imaging(SWI)with conventional sequence,DWI and QSM images were retrospectively analyzed,and the differences of CMBs detected by different sequences and the distribution characteristics of lesions were compared.Results:The distribution of CMBs were predominately located in subcortical area and basal ganglia region.SWI and QSM could clearly show the morphological characteristics of CMBs.CMBs positive detected by QSM,conventional and DWI sequence had significant difference(χ^(2)=48.49,P<0.001).The number of CMBs detected by SWI,QSM and conventional sequence had significant difference(χ^(2)=43.40,P<0.001).No statistical difference was found in different brain areas for the number of CMBs between SWI and QSM sequences(χ^(2)=8.72,P=0.07).Conclusion:QSM sequence has high accuracy in recognition and identification of CMBs,which can be used as a supplement to SWI sequence and has certain clinical application value.
