股骨近端髓内钉-螺旋刀片内固定在老年髋部骨折中的疗效及对骨代谢的影响

摘　　要：目的:探究与观察股骨近端髓内钉-螺旋刀片内固定在老年髋部骨折中的疗效及对骨代谢的影响。方法:选取2018年6月-2020年10月辽阳辽化医院收治的90例老年髋部骨折患者为研究对象,将其随机分为对照组(动力髋螺钉)和观察组(股骨近端髓内钉-螺旋刀片内固定),每组45例。比较两组术后不同时间的Harris评分、肿胀消退时间、骨折愈合时间、并发症发生率、手术前后的骨代谢指标及结构指标。结果:术后1、3、6个月,观察组的Harris评分结果均显著优于对照组,肿胀消退时间及骨折愈合时间均显著短于对照组,并发症发生率显著低于对照组,差异均有统计学意义(P<0.05)。术前,两组的骨代谢指标及结构指标比较,差异均无统计学意义(P>0.05);术后1、2、4周,观察组的骨代谢指标均显著优于对照组(P<0.05);术后1、3、6个月,观察组的结构指标均显著优于对照组,差异均有统计学意义(P<0.05)。结论:股骨近端髓内钉-螺旋刀片内固定在老年髋部骨折中的疗效较好,且对骨代谢的影响更为积极,因此在老年髋部骨折患者中的应用价值较高。Objective:To investigate and observe the effect of proximal femoral nail with spiral blade fixation in the elderly patients with hip fracture and its influence for bone metabolism.Method:A total of 90 elderly patients with hip fracture admitted to Liaoyang Liaohua Hospital from June 2018 to October 2020 were selected as the research objects,and they were randomly divided into control group(dynamic hip screw)and observation group(proximal femoral nail with spiral blade fixation),with 45 cases in each group.Then the Harris score at different time after the operation,swelling subsided time,fracture healing time,complication rates,bone metabolism indexes and structure indexes before and after the operation of two groups were compared.Result:At 1,3 and 6 months after surgery,the Harris scores of the observation group were significantly better than those of the control group,the time of swelling regression and fracture healing were significantly shorter than those of the control group,and the incidence of complications was significantly lower than that of the control group,the differences were statistically significant(P<0.05).Before surgery,there were no significant differences in bone metabolism and structural indexes between the two groups(P>0.05);at 1,2 and 4 weeks after surgery,bone metabolism indexes in observation group were significantly better than those in control group(P<0.05);at 1,3 and 6 months after surgery,the structural indexes of the observation group were significantly better than those of the control group,the differences were statistically significant(P<0.05).Conclusion:The effect of proximal femoral nail with spiral blade fixation in the elderly patients with hip fracture is better,and its influence for bone metabolism is more active,so its application value in the elderly patients with hip fracture is higher.
