肺炎支原体肺炎患儿血清急性时相反应蛋白及血小板活化指标的变化观察

摘　　要：目的:探讨肺炎支原体肺炎患儿血清急性时相反应蛋白及血小板活化指标的变化,为临床决策提供参考。方法:以2018年10月-2021年2月辽宁省健康产业集团阜新矿总医院收治的82例确诊为肺炎支原体肺炎患儿为研究对象,根据疾病的严重程度分为轻症组42例和重症组40例,并以同期健康体检的42例健康儿童作为对照组。用酶联免疫吸附法检测血清急性时相反应蛋白(C反应蛋白、α1-酸性糖蛋白、结合珠蛋白、铜蓝蛋白)和血小板活化指标[D-二聚体(D-D)、可溶性P-选择素、血小板活化因子(PAF)],全自动血细胞分析仪检测血小板计数(PLT)。结果:轻症组和重症组的C反应蛋白、α1-酸性糖蛋白、结合珠蛋白和铜蓝蛋白均高于对照组(P<0.05),重症组C反应蛋白、α1-酸性糖蛋白、结合珠蛋白和铜蓝蛋白均高于轻症组(P<0.05)。轻症组和重症组的D-D、可溶性P-选择素、PAF均高于对照组,重症组的D-D、可溶性P-选择素、PAF均高于轻症组(P<0.05);轻症组和重症组的PLT均低于对照组,重症组低于轻症组,差异均有统计学意义(P<0.05)。C反应蛋白、α1-酸性糖蛋白、结合珠蛋白、铜蓝蛋白、D-D、可溶性P-选择素、PAF均与疾病严重程度呈正相关(P<0.05),PLT与疾病严重程度呈负相关(P<0.05)。结论:血清急性时相反应蛋白和血小板活化指标与疾病的严重程度密切相关,为临床诊断、监测和预后疾病的发生发展提供指导性依据。Objective:To investigate the changes of serum levels of acute phase reaction protein and platelet activation indexes in children with mycoplasma pneumoniae pneumonia,and to provide reference for clinical decision-making.Method:A total of 82 children diagnosed with mycoplasma pneumoniae pneumonia admitted to Fuxin Mining General Hospital of Liaoning Health Industry Group from October 2018 to February 2021 were selected as the research subjects.According to the severity of the disease,they were divided into mild group(n=42)and severe group(n=40),42 healthy children who underwent physical examination in the same period were selected as the control group.Serum acute phase reaction protein(C-reactive protein,α1-acid glycoprotein,binding globin and ceruloplasin)and platelet activation indexes[D-dimer(D-D),soluble P-selectin,platelet activation factor(PAF)]were determined by enzyme-linked immunosorbent assay.Platelet count(PLT)was determined by automatic blood cell analyzer.Result:The levels of C-reactive protein,α1-acid glycoprotein,binding globin and ceruloplasmin in mild and severe group were higher than those in control group(P<0.05).The levels of C-reactive protein,α1-acid glycoprotein,binding globin and ceruloplasmin in severe group were higher than those in mild group(P<0.05).The levels of D-D,soluble P-selectin and PAF in the mild and severe group were higher than those in the control group(P<0.05),the levels of D-D,soluble P-selectin and PAF in the severe group were higher than those in the mild group(P<0.05).The PLT of the mild and severe groups was lower than that of the control group,and that of the severe group was lower than that of the mild group,the differences were statistically significant(P<0.05).C-reactive protein,α1-acidic glycoprotein,globin binding,ceruloplasmin,D-D,soluble P-selectin and PAF were positively correlated with disease severity(P<0.05),while PLT was negatively correlated with disease severity(P<0.05).Conclusion:Acute phase reaction protein and platelet activation indexes are close
