摘 要:目的:探讨改良式三球呼吸训练仪在胸部手术后肺康复中的应用效果。方法:选择2020年1月-2021年1月在萍乡市人民医院胸外科行手术治疗的60例患者作为研究对象,根据手术时间先后进行编号,采用随机数字表法将其分为观察组(n=30)和对照组(n=30)。对照组在术后进行常规气球肺康复,观察组使用改良三球呼吸训练仪进行康复。比较两组干预前后肺功能指标[第一秒用力呼气量(FEV_(1))、用力肺活量(FVC)、第一秒用力呼气量占用力肺活量比率(FEV_(1)/FVC%)]、运动耐力、锻炼期间呼吸频率及血气分析指标[氧分压(PaO_(2))、二氧化碳分压(PaCO_(2))]。结果:干预后,两组肺功能指标均优于干预前,且观察组FEV_(1)、FVC均高于对照组,差异均有统计学意义(P<0.05),但两组FEV_(1)/FVC%比较,差异无统计学意义(P>0.05)。干预后,两组6 min步行距离均远于干预前,且观察组远于对照组,差异均有统计学意义(P<0.05)。干预后,两组血气指标均优于干预前,且观察组呼吸频率、PaCO_(2)均低于对照组,PaO_(2)高于对照组,差异均有统计学意义(P<0.05)。结论:在胸部手术后患者肺康复训练过程中使用改良式三球呼吸训练仪能进一步提高患者肺功能,改善运动耐量与血气指标。Objective:To explore the application effect of improved three-ball breathing training device in pulmonary rehabilitation after chest surgery.Method:Sixty patients who underwent surgical treatment in Pingxiang People’s Hospital from January 2020 to January 2021 were selected as the research objects.They were numbered according to the operation time,and they were divided into the observation groups(n=30)and the control group(n=30)by random number table method.The control group was given routine balloon lung rehabilitation after operation,and the observation group was given modified three-ball breathing training device for rehabilitation.The lung function indexes[the first second forced expiratory volume(FEV_(1)),forced vital capacity(FVC),forced expiratory volume in the first second occupancy force vital capacity ratio(FEV_(1)/FVC%)],exercise endurance,breathing frequency and blood gas analysis indicators[partial oxygen pressure(PaO_(2)),partial pressure of carbon dioxide(PaCO_(2))]of two groups before and after intervention were compared.Result:After intervention,the lung function indexes in both groups were better than those before intervention,and the FEV_(1) and FVC in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05),but there was no significant difference in FEV_(1)/FVC%between two groups(P>0.05).After intervention,the 6-min walking distance in both groups were much longer than those before intervention,and the observation group was much longer than that in the control group,the differences were statistically significant(P<0.05).After intervention,the blood gas indexes in both groups were better than those before intervention,and the respiration rate and PaCO_(2) in the observation group were lower than those in the control group,while PaO_(2) was higher than that in the control group,the differences were statistically significant(P<0.05).Conclusion:The use of modified three-ball breathing training device during pulmonary rehabilitation
