耳穴压豆联合腹部按摩对高龄骨折术后拔除尿管后预防尿潴留的护理研究

摘　　要：目的:探讨耳穴压豆联合腹部按摩对高龄骨折术后拔除尿管后预防尿潴留的应用效果。方法:选取2020年12月-2021年4月南昌市洪都中医院脊柱关节科收治的80例高龄股骨颈骨折手术患者。按随机数字表法分为两组,每组40例。对照组于拔除尿管前行常规护理干预,观察组在对照组的基础上给予耳穴压豆联合腹部按摩干预。比较两组拔除尿管后首次排尿情况、首次排尿时间及尿量、尿潴留及护理满意度情况。结果:观察组拔除尿管后首次排尿总有效率为95.00%,高于对照组的80.00%(P<0.05);观察组首次排尿时间为(3.68±0.52)h,短于对照组的(4.96±0.75)h,首次排尿量为(315.41±45.62)mL,多于对照组的(253.17±40.18)mL,差异均有统计学意义(P<0.05);观察组尿潴留发生率为5.00%,低于对照组的20.00%(P<0.05);观察组护理满意度为92.50%,高于对照组的75.00%(P<0.05)。结论:耳穴压豆联合腹部按摩可有效降低高龄骨折术后拔除尿管后尿潴留发生率,缩短患者首次排尿时间,增加排尿量,利于提高整体护理满意度。Objective:To explore the effect of ear acupoint pressing beans combined with abdominal massage on prevention of urinary retention after removal of urinary catheter after fracture surgery in elderly patients.Method:A total of 80 elderly patients with femoral neck fracture surgery who admitted to the Department of Spine and Arthritis in Nanchang Hongdu Hospital of Traditional Chinese Medicine from December 2020 to April 2021 were selected and divided into two groups according to a random number table,40 cases in each group.The control group was given routine nursing intervention before removal of urinary catheter,the observation group was given ear acupoint pressing beans combined with abdominal massage intervention on the basis of the control group.The situation of first urination,first urination time and urine volume,urinary retention and nursing satisfaction were compared between two groups after removal of urinary catheter.Result:The total effective rate of the first urination after removal of urinary catheter in the observation group was 95.00%,which was higher than 80.00%in the control group(P<0.05);the first urination time in the observation group was(3.68±0.52)h,which was shorter than(4.96±0.75)h in the control group,the first urinary volume was(315.41±45.62)mL,which was more than(253.17±40.18)mL in the control group,the differences were statistically significant(P<0.05);the incidence of urinary retention in the observation group was 5.00%,which was lower than 20.00%of the control group(P<0.05);the nursing satisfaction was 92.50%in the observation group,which was higher than 75.00%the control group(P<0.05).Conclusion:Ear acupoint pressing beans combined with abdominal massage can effectively reduce the incidence of urinary retention after removal of urinary catheters in elderly patients after fracture operation,shorten the time of first urination,increase urination output,and improve overall nursing satisfaction.
