摘 要:目的:探讨新冠肺炎疫情期间新冠肺炎患者和一线医护人员创伤后应激障碍(PTSD)发生率及其影响因素。方法:采用创伤后应激障碍检查表(PCL),借助微信平台交流群和网络问卷对2020年1—3月在宁夏第四人民医院确诊后收治的59例新冠肺炎患者以及187例一线医护人员进行调查,采用多因素logistic回归分析PTSD的影响因素。结果:246例研究对象PTSD平均得分为(28.57±11.72)分,PTSD发生率为16.7%(41例);医护人员和新冠肺炎患者PTSD发生率分别为11.2%(21例)和33.9%(20例),两者PTSD平均得分差异有统计学意义(P=0.001)。多因素logistic回归分析显示,女性(OR=3.855)、高警觉反应(OR=1.156)和创伤后再体验高得分(OR=1.208)是发生PTSD的危险因素(P<0.01)。结论:新冠肺炎疫情期间患者和一线医护人员均存在不同程度的PTSD,应积极给予其心理疏导,以减少和消除PTSD的发生。Objective:To investigate the incidence and influencing factors of post-traumatic stress disorder(PTSD)in COVID-19 patients and front-line medical staff during the epidemic period of COVID-19.Methods:A total of 59 COVID-19 patients and 187 front-line medical staff admitted to Ningxia Fourth People's Hospital from January to March 2020 were investigated by post-traumatic stress disorder checklist(PCL)through WeChat platform exchange groups and network questionnaire.The influencing factors of PTSD were analyzed by multivariate logistic regression.Results:The mean PTSD score of 246 study subjects was(28.57±11.72)points and the incidence of PTSD was 16.7%(41 cases);the incidences of PTSD in medical staff and COVID-19 patients were 11.2%(21 cases)and 33.9%(20 cases),respectively,and there was a significant difference in the mean PTSD score between the two groups(P=0.001).Multivariate logistic regression analysis showed that female gender(OR=3.855),high score of hypervigilance response(OR=1.156)and post-traumatic re-experience(OR=1.208)were the risk factors for developing PTSD(P<0.01).Conclusion:Both COVID-19 patients and front-line medical staff have different degrees of PTSD during the epidemic period of COVID-19,which suggests that psychological counseling should be actively given to reduce and eliminate the occurrence of PTSD in COVID-19 patients and front-line medical staff.
