摘 要:目的研究语后聋成年人工耳蜗植入者术后生活质量随时间变化的发展趋势和影响因素。方法对28例语后聋成年人工耳蜗植入者(男16例,女12例,年龄39.82±16.38岁)分别在人工耳蜗植入术前、开机后1、3、6、12个月时,采用Nijmegen人工耳蜗植入量表(NCIQ中文版,包括基本声音感知、高级声音感知、言语能力、自信心、活动能力和社会交往6个维度)评估其生活质量。针对开机后1年的生活质量得分,分析植入年龄、耳聋时长、助听器使用时长及性别、受教育程度、居住地对NCIQ总分及各子维度得分的影响。结果开机1年内随着人工耳蜗使用时间的延长,NCIQ总分及各子维度得分均呈现上升趋势,其中NCIQ生活质量总分(β=1.039,t=3.59,P=0.0005)、基本声音感知(β=2.431,t=5.85,P<0.0001)、高级声音感知(β=1.843,t=5.02,P<0.0001)和言语能力(β=1.049,t=2.97,P=0.0035)随使用时间延长均有显著上升的趋势;而其他3个子维度(自信心、活动能力和社会交往)得分随时间延长没有显著性增长(P>0.05)。植入年龄仅与开机后1年NCIQ总分(P=0.039)及言语能力(P=0.004)呈负相关性,耳聋时长仅与自信心(P=0.035)呈正相关性,使用助听器时长与所有子维度得分及总分相关性均不明显(P>0.05)。结论语后聋成年人工耳蜗植入者术后生活质量随CI使用时间延长而提高,其中生理功能(基本声音感知、高级声音感知和言语能力)显著改善;植入年龄、耳聋时长、助听器使用时长及性别、受教育程度、居住地对生活质量改善无明显影响。Objective To evaluate the progression and influencing factors of quality of life in postlingually deafened adult cochlear implant users over time.Methods Twenty-eight postlingually deafened adult cochlear implant users,16 males and 12 females with the average age of 39.82±16.38 years old,participated during a prospective follow-up evaluation.Nijmegen cochlear implant questionnaire(NCIQ)was translated into Chinese version and used to assess the quality of life after implantation.There were six subdomains in NCIQ,including basic sound perception,advanced sound perception,speech production,self-esteem,activities and social interactions.The questionnaire was administered at the 1st,3rd,6th and 12th month after the CI switch-on.We also analyzed the effects of age at implantation,duration of deafness,duration of hearing-aid use,gender,education level and residence on the scores of NCIQ.Results The performance of quality of life demonstrated an increasing trend in outcome during the entire 1-year follow-up process.The total scores of NCIQ questionnaire revealed a significant improvement(β=1.039,t=3.59,P=0.0005),and 3 subdomains showed obvious improvement(basic sound perception,β=2.431,t=5.85,P<0.0001;advanced sound perception,β=1.843,t=5.02,P<0.0001;speech production,β=1.049,t=2.97,P=0.0035).There was no significant improvement in self-esteem,activities and social interaction(P>0.05).There was a significant negative correlation between age at implantation and total score of NCIQ(P=0.039)and speech production(P=0.004),and a positive correlation between duration of deafness and self-esteem(P=0.035).Conclusion The quality of life of postlingually deafened adult cochlear implant patients progressed during the 1-year follow-up evaluation,especially their physiological functions(basic sound perception,advanced sound perception and speech ability)significantly improved.Age at implantation,duration of deafness,duration of hearing-aid use,gender,education level and residence had no obvious effects on the improvement of qual
