重庆市首例人感染H5N6禽流感病例流行病学调查分析

摘　　要：目的分析重庆市首例人感染H5N6禽流感病例流行病学特征,提高各级医疗机构对"不明原因肺炎"的报告能力,为防制人感染禽流感提供科学依据。方法于2021-01对重庆首例人感染H5N6禽流感病例、密切接触者、可疑暴露者开展现场流行病学调查,采集生物和环境及食品样本进行实验室检测,运用描述流行病学进行分析。结果重庆市首例确诊人感染H5N6禽流感病例,咽拭子样本检测结果为H5N6亚型禽流感病毒阳性;34名密切接触者和19名可疑暴露人员未发生H5N6禽流感传播;首例病人有家禽和活禽市场暴露史,日常买菜的活禽市场和活禽市场禽类来源地H5亚型禽流感病毒核酸阳性,活禽市场多处检测出禽流感病毒。结论重庆市首例人感染H5N6禽流感病例为本地感染,感染来源可能与接触市场活禽有关,非人传人病例。Objective To analyze the epidemiological characteristics of the first case of human infection with H5N6 avian influenza in Chongqing so as to improve the ability of medical institutions to report“unexplained pneumonia”and provide scientific basis for the prevention and control of human infection with avian influenza.Methods The first case of human infection with H5N6 avian influenza,close contacts and suspected exposure people were investigated from January 2021.Biological and environmental samples were collected for laboratory test,and the descriptive epidemiological method was used to analyze the source of the infection.Results The first human case of avian influenza A(H5N6)in Qingshi City has been confirmed by a positive result for the H5N6 subtype of avian influenza virus in throat swab samples.No H5N6 avian influenza transmission occurred in 34 close contacts and 19 suspected exposed people.The case had a history of exposure to poultry and live poultry markets.The nucleic acid of H5 subtype avian influenza virus was positive in the live poultry market and the origin places of poultry.Avian influenza virus was detected in many places in the live poultry market.Conclusions The first human case of H5N6 avian influenza in Chongqing is local infection.The source of infection may be related to live poultry in the market,and human to human transmission was not found.
