摘  要:目的观察卒中后认知障碍(PSCI)患者血清微小RNA(miRNAs)表达谱,验证目标miRNAs组合的表达情况及加味涤痰汤对其影响。方法通过高通量测序方式测定PSCI患者及对照组(脑卒中后无认知障碍组和正常人组)血清miRNA表达情况,筛选与PSCI发病过程密切相关差异表达的miRNAs组合进行研究。收集30例PSCI患者给予加味涤痰汤口服,观察疗程8周,另收集同期认知功能正常的老年健康体检者30例作为对照组。采用荧光定量PCR法检测PSCI患者治疗前后及对照组血清目标miRNAs的水平。结果对筛选的20个差异miRNAs进行研究。与对照组比较,PSCI组治疗前血清miR-31、miR-93、miR-143、miR-146a、miR-107、miR-126表达显著升高(P<0.05),miR-29a,miRNA-34a表达显著降低(P<0.05)。与治疗前比较,PSCI组治疗后血清miR-29a、miRNA-34a表达升高(P<0.05),miR-31、miR-93、miR-143、miR-146a、miR-126表达显著降低(P<0.05),miR-107表达降低(P<0.05)。结论通过检测血清miR-31、miR-93、miR-143、miR-146a、miR-107、miR-126及miR-29a、miRNA-34a表达水平可能有助于PSCI患者的早期诊断;加味涤痰汤能升高PSCI患者外周血血清miR-29a、miRNA-34a的水平,显著降低miR-31、miR-93、miR-143、miR-146a、miR-126的水平,使miR-107水平降低。Objective To observe the expression profile of serum microRNAs(miRNAs)in patients with post-stroke cognitive impairment(PSCI),and to verify the expression of the target miRNAs combination and the eff ect of modifi ed Ditan Decoction on it.Methods The expression of serum miRNA in the PSCI patients and the control group(a group of patients without cognitive impairment after stroke)was measured by the high-throughput sequencing,and the diff erentially expressed miRNAs combination closely related to the pathogenesis of PSCI was screened for the study.A total of 30 PSCI patients were collected and given modifi ed Ditan Decoction orally for 8 weeks,and another 30 healthy elderly patients with normal cognitive function during the same period were collected as the control group.The levels of target miRNAs in serum of PSCI patients before and after treatment and the control group were detected by fluorescence quantitative PCR.Results The screened 20 differential miRNAs were studied.Compared with the control group,the serum expressions of miR-31,miR-93,miR-143,miR-146a,miR-107 and miR-126 in the PSCI group were signifi cantly higher before treatment(P<0.05),while the expressions of miR-29a and miRNA-34a were signifi cantly lower(P<0.05).Compared with before treatment,the expressions of serum miR-29a and miRNA-34a in the PSCI group were increased(P<0.05),while the expressions of miR-31,miR-93,miR-143,miR-146a and miR-126 were signifi cantly decreased(P<0.05),and the expression of miR-107 was decreased(P<0.05).Conclusion The detection of serum miR-31,miR-93,miR-143,miR-146a,miR-107,miR-126 and miR-29a,miR-34a expression levels may be helpful for the early diagnosis of PSCI patients.Modifi ed Ditan Decoction can increase the levels of miR-29a and miRNA-34a in peripheral blood serum of PSCI patients,signifi cantly reduce the levels of miR-31,miR-93,miR-143,miR-146a and miR-126,and reduce the level of miR-107.
