葛根芩连汤加味对溃疡性结肠炎患者营养状况、T淋巴细胞亚群、CRP及ESR的影响

摘　　要：目的:探讨葛根芩连汤加味对溃疡性结肠炎(UC)患者营养状况、T淋巴细胞亚群、C反应蛋白(CRP)及血沉(ESR)的影响。方法:选取符合纳入标准的62例UC患者,按简单随机数字表法随机分为观察组及对照组,每组各31例。对照组接受常规治疗,观察组在此基础上口服葛根芩连汤加味治疗,两组均连续治疗4周。比较两组患者治疗前后营养状况指标血清白蛋白(ALB)、前白蛋白(PA)、转铁蛋白(TRF),T淋巴细胞亚群CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+),CRP及ESR水平,以及两组患者临床疗效和不良反应发生情况。结果:观察组临床总有效率显著优于对照组(P<0.05)。治疗前,两组患者ALB、PA、TRF、CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)、CRP和ESR水平比较差异无统计学意义(P>0.05)。治疗后,两组患者ALB、PA、TRF、CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平均高于治疗前,CD8^(+)、CRP、ESR水平均低于治疗前,且观察组优于对照组(P<0.05)。两组患者不良反应发生率比较差异无统计学意义(P>0.05)。结论:葛根芩连汤加味治疗UC疗效显著,可明显改善患者营养状况、T淋巴细胞亚群、CRP及ESR水平,且不良反应发生率较低,安全性尚可,值得临床推广。Objective:To explore the effects of modified Gegen Qinlian Decoction on nutritional status,T lymphocyte subsets,C-reactive protein(CRP)and erythrocyte sedimentation rate(ESR)in patients with ulcerative colitis(UC).Methods:62 patients with UC were randomly divided into the observation group and the control group,with 31 patients in each group.The control group received conventional treatment;on which basis,the observation group received modified Gegen Qinlian Decoction.Both groups were treated for 4 consecutive weeks.Nutritional status indicators of serum albumin(ALB),prealbumin(PA)and transferrin(TRF),T lymphocyte subsets of CD3^(+),CD4^(+),CD8^(+)and CD4^(+)/CD8^(+),as well as the levels of CRP and ESR were compared between before and after the treatment in the two groups.Clinical efficacy and adverse reactions were also compared between the two groups.Results:The total effective rate of the observation group was significantly higher than that of the control group(P<0.05).There were no statistical differences in the levels of ALB,PA,TRF,CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+),CRP and ESR between the two groups before the treatment(P>0.05).The levels of ALB,PA,TRF,CD3^(+),CD4^(+),CD4^(+)/CD8^(+)were higher,whereas the levels of CD8^(+),CRP and ESR were lower after the treatment than those before the treatment in the two groups(P<0.05);of which the improvements in the observation group were more significant(P<0.05).There was no statistical difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Modified Gegen Qinlian Decoction has a significant effect in the treatment of UC,it can significantly improve the nutritional status,T lymphocyte subsets and the levels of CRP and ESR,with less adverse reactions and fair safety,which is worthy of clinical promotion.
