家庭赋权护理在肝硬化失代偿期病人中的应用

摘　　要：目的:探讨家庭赋权护理对肝硬化失代偿期病人及家属的影响。方法:选取肝硬化失代偿期病人80例,按照随机数字表法分为对照组和赋权组,各40例。对照组实施常规护理,赋权组在常规护理基础上,应用家庭赋权护理干预。比较2组病人护理前后生活质量、心理状况和家属负荷、综合照顾能力。结果:干预后,赋权组病人汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表评分(HAMD)均低于对照组(P<0.01和P<0.05),生活质量各维度评分均高于对照组(P<0.05~P<0.01);赋权组病人家属照顾负荷各项评分均低于对照组(P<0.05~P<0.01),综合照顾能力各项评分均明显高于对照组(P<0.01)。结论:家庭赋权护理有助于改善肝硬化失代偿期病人的心理状况和生活质量,减轻家属照顾负荷,提高家属综合照顾能力。Objective:To explore the effects of the family empowerment nursing on patients with decompensated stage of liver cirrhosis and their families.Methods:Eighty patients with decompensated stage of liver cirrhosis were randomly divided into the control group and empowerment group(40 case in each group).The control group was nursed with routine nursing method,and the empowerment group was additionally nursed with family empowerment nursing on the basis of routine nursing.The quality of life,psychological status,family load and comprehensive care ability in two groups were compared between before and after nursing.Results:After intervention,the scores of Hamilton anxiety scale(HAMA)and Hamilton depression scale(HAMD)in empowerment group were significantly lower than those in control group(P<0.01 and P<0.05),and the dimensions score of quality of life in empowerment group were significantly higher than those in control group(P<0.05 to P<0.01).The scores of family care load in empowerment group were lower than those in control group(P<0.05 to P<0.01),and the score of comprehensive care ability in empowerment group was significantly higher than that in control group(P<0.01).Conclusions:The family empowerment nursing can significantly improve the psychological status and quality of life of patients with decompensated stage of liver cirrhosis,reduce the load of family members and improve their comprehensive care ability.
