观察CT引导定位针刀松解病变小关节治疗腰椎小关节病疗效

摘　　要：目的:观察CT引导定位下腰椎小关节病应用针刀松解病变小关节治疗的疗效。方法:于2018年1月-2020年1月,选取腰椎小关节病患者60例,根据其就诊单双号组别,单号30例为对照组,双号30例为试验组。采用单纯脊柱定点旋转手法治疗对照组,在CT引导定位下采用针刀松解病变小关节治疗试验组。比较两个组别Oswestry功能障碍指数(Oswestry Disability Index,ODI)、视觉模拟评分(Visual Analogue Scale,VAS)等评分变化、椎体移位情况、总有效率及生活质量。结果:两个组别治疗后ODI、VAS等评分、椎体位移等对比发现试验组均明显优于对照组(P<0.05),差异有统计学意义;但二者治疗前差异无统计学意义(P>0.05)。两个组别总有效率对比发现试验组93.33%(28/30)明显高于对照组80.0%(24/30)(P<0.05),有统计学价值。两个组别各因子生活质量评分对比发现试验组均高于对照组(P<0.05),差异有统计学意义。结论:CT引导定位下腰椎小关节病应用针刀松解病变小关节治疗的效果确切,即可有效减轻患者腰部疼痛,改善腰椎功能,还能缩小椎体移位,提高其生活质量,值得推广研究。Objective:To observe the efficacy of acupotomology in the treatment of lumbar arthropathies with CT guided localization.Methods:From January 2018 to January 2020,60 patients with lumbar arthropathy were selected.According to the group of odd-even number,30 patients with odd number were in the control group and 30 patients with even number were in the experimental group.The control group was treated with pure fixed-point rotation manipulation,and the experimental group was treated with acupotomology under CT guided positioning.The changes of Oswestry disability index(ODI)and visual analogue scale(VAS)scores,vertebral displacement,total efficiency and quality of life were compared between the two groups.Results:After treatment,ODI,VAS,and vertebral displacement were compared between the two groups,and it was found that the experimental group was significantly better than the control group(P<0.05),which was with statistical significance.However,there was no significant difference between them before treatment(P>0.05).The total effective rate of the experimental group was 93.33%(28/30),which was significantly higher than 80.0%(24/30)of the control group(P<0.05),the difference was of statistical significance.The quality of life scores of each factor in the experimental group was higher than that in the control group(P<0.05),the difference was of statistical significance Conclusion:CT guided treatment of lumbar arthropathy by acupotomology is effective in reducing lumbar pain,improving lumbar function,reducing vertebral displacement,and improving the quality of life,which is worthy of popularization and research.
