摘 要:目的研究神经内镜超早期微创术联合重组人促红细胞生成素(rHu-EPO)对老年重症高血压脑出血患者S100钙结合蛋白A8/A9复合物(S100A8/A9)、髓鞘碱性蛋白(MBP)水平的影响。方法以海安市人民医院2016年1月~2019年5月收治的120例高血压脑出血患者为研究对象,按照随机数表法分为观察组和对照组,每组60例。对照组采取神经内镜超早期微创术治疗,观察组在对照组的基础上联合采用rHu-EPO治疗。比较两组围术期指标、神经损伤情况、S100A8/A9、MBP水平以及并发症的发生情况。结果观察组住院时间低于对照组(P<0.05);两组患者的神经损伤各项评分均高于治疗前,且观察组高于对照组(P<0.05);两组S100A8/A9、MBP均低于治疗前,且观察组低于对照组(P<0.05);观察组患者的重症肺炎、颅内感染、消化道出血、泌尿系统感染以及下肢静脉血栓发生率均低于对照组(P<0.05)。结论应用神经内镜超早期微创术联合rHu-EPO可显著降低老年重症高血压脑出血患者S100A8/A9、MBP水平,治疗效果显著。Objective To study the effect of neuroendoscopic minimally invasive surgery combined with recombinant human erythropoietin(rHu-EPO) on the levels of S100 A8/A9 and myelin basic protein(MBP) in older patients with severe hypertensive cerebral hemorrhage.Methods A total of 120 patients with hypertensive cerebral hemorrhage from January 2016 to May 2019 in Hai’an People’s Hospital were enrolled in this study.The patients were randomly divided into the observation group and the control group, 60 cases in each group.The patients in both groups were treated with neuroendoscopic minimally invasive surgery, and the patients in the observation group were additionally treated with rHu-EPO.The perioperative indices, nerve injury, the levels of S100 A8/A9 and MBP,and complications between the two groups were compared.Results The length of stay in the observation group was significantly shorter than that in the control group(P<0.05).The scores of nerve injury in the two groups were higher than those before treatment, and those in the observation group were higher than those in the control group(P<0.05).The levels of S100 A8/A9 and MBP in the two groups were lower than those before treatment, and those in the observation group were lower than those in the control group(P<0.05).The incidences of severe pneumonia, intracranial infection, gastrointestinal bleeding, urinary system infection and lower extremity venous thrombosis in the observation group were significantly lower than those in the control group(P<0.05).Conclusion Neuroendoscopic minimally invasive surgery combined with rHu-EPO might significantly reduce the levels of S100 A8/A9 and MBP,and improve the therapeutic effect in older patients with severe hypertensive cerebral hemorrhage.
