11例头癣患者皮肤镜特点回顾性分析

摘　　要：目的探讨皮肤镜下逗号发、螺旋状发、营养不良发头癣特征性表现规律,找出皮肤镜初步鉴别犬小孢子菌和断发毛癣菌方法。方法选取2019—2021年广西医科大学第一附属医院皮肤科首诊真菌涂片为阳性的11例头癣患者,对其皮肤镜特点进行回顾性分析。结果在犬小孢子菌感染的白癣患者中逗号发100%,营养不良发80%,无螺旋状发;在断发毛癣菌感染的头癣中,逗号发和螺旋状发100%,营养不良发66%。如白癣患者同时出现营养不良发,逗号发和螺旋状发提示断发毛癣菌感染的可能(χ^(2)=8.00,P=0.180);如只出现营养不良发,逗号发提示犬小孢子菌感染的可能。结论螺旋状发对于犬小孢子菌和断发毛癣菌鉴别具有特异性,在头癣患者真菌培养前,皮肤镜对头癣中犬小孢子菌和断发毛癣菌感染的诊断有一定意义。Objective:To explore the dermoscopic characteristics of comma,spiral,malnourished hair,and to find out a new method for differentiation of Microsporum canis and Trichophyton tonsurans infections under the dermoscope.Methods:Dermoscopic characteristics were analyzed in 11 patients with tinea capitis who were positive for fungal smear and underwent dermoscopy at department of dermatology of the First Affiliated Hospital of Guangxi Medical University from 2019 to 2021.Results:Comma hair and malnourished hair were 100%and 80%,respectively,with no spiral hair,in patients infected by Microsporum canis.All patients with Trichophyton tonsurans infections had comma and spiral hair,and 66%had malnourished hair.That a patient with tinea capitis had comma,spiral,malnourished hair at the same time highly indicated Trichophyton tonsurans infections(χ^(2)=8.00,P=0.18).Conclusions:Comma,spiral,and malnourished hairs are specific markers for the differentiation of Microsporum canis and Trichophyton tonsurans.Dermoscopy is a valuable approach in the diagnosis of Microsporum canis and Trichophyton tonsurans prior to fungal culture in patients with tinea capitis.
