应用静脉剥脱导管手术与传统手术治疗原发性大隐静脉曲张的疗效对比

摘　　要：目的探讨静脉剥脱导管手术与大隐静脉高位结扎+剥脱术治疗原发性大隐静脉曲张的近期疗效差异。方法收集2019年8月至2020年7月南通市中医院收治的80例单侧大隐静脉曲张患者的临床资料,根据手术方法的不同将患者分为静脉剥脱导管组(n=40,应用静脉剥脱导管进行手术)和传统手术组(n=40,采用大隐静脉高位结扎+剥脱术)。术后,随访12个月,比较两组患者术后复发情况及并发症发生情况。结果术后,两组患者下肢深静脉血栓形成、切口皮肤感染、皮肤麻木感比较,差异均无统计学意义(P>0.05)。静脉剥脱导管组患者皮下瘀血的发生率为10%(4/40),低于传统手术组患者的27.5%(11/40),差异有统计学意义(P=0.045)。术后,静脉剥脱导管组患者的复发率为2.5%(1/40),低于传统手术组患者的20.0%(8/40),差异有统计学意义(P=0.025)。结论静脉剥脱导管治疗大隐静脉曲张具有安全、有效、操作简单、并发症少等特点,具有临床推广价值。Objective To investigate the difference of short-term curative effect between venous stripping catheter operation and high saphenous vein ligation+stripping in the treatment of primary varicosis of great saphenous vein.Method The clinical data of 80 patients with unilateral varicosis of great saphenous vein admitted to Nantong Hospital of Traditional Chinese Medicine from August 2019 to July 2020 were collected,and the patients were divided into the venous stripping catheter group(n=40,surgery with venous stripping catheter)and the traditional operation group(n=40,using high saphenous vein ligation+stripping)according to different surgical methods.After 12 months of follow-up,the postoperative recurrence and complications were compared between the two groups.Result After the operation,there was no significant difference in lower extremity deep vein thrombosis,incision skin infection,and skin numbness between the two groups(P>0.05).The incidence of subcutaneous blood stasis in the venous stripping catheter group was 10%(4/40),which was lower than 27.5%(11/40)in the traditional surgery group,and the difference was statistically significant(P=0.045).After the operation,the recurrence rate of the patients in the vein stripping catheter group was 2.5%(1/40),which was lower than 20.0%(8/40)in the traditional surgery group,and the difference was statistically significant(P=0.025).Conclusion The employment of vein stripping catheter,which has clinical promotion value,for varicose vein characterize with safety,effectiveness,simple to operate,and fewer risk of complications.
