一例患者痰液白喉棒状杆菌伴纹带棒状杆菌分离培养与鉴定分析

摘　　要：目的对福建省1例疑似白喉住院患者进行病原学检测,以明确诊断。方法采集患者痰标本进行分离培养,利用生化鉴定,基质辅助激光解吸电离飞行时间质谱(MALDI-TOF MS)鉴定以及白喉相关基因(16SrRNA、dtxR、toxA、toxB)检测。结果在患者痰液中分离到白喉棒状杆菌,其生化检测、质谱鉴定和16srRNA测序比对结果均符合白喉棒状杆菌特征,菌株dtxR、toxA和toxB基因均为阴性;同时,检出的纹带棒状杆菌经生化反应、质谱鉴定符合纹带棒状杆菌鉴定结果。结论该病例不携带白喉毒力基因,为无毒白喉棒状杆菌带菌者。基质辅助激光解析电离-飞行时间质谱技术,能快速鉴定区分白喉棒状杆菌与纹带棒状杆菌,为临床快速诊断提供依据。Objective To confirm the diagnosis of a hospitalized suspect case of diphtheria in Fujian Province by etiological detection.Methods The sputum samples from the patient were isolated and cultured.The isolated strains were identified by biochemical identification and MALDI-TOF MS method and Corynebacterium diphtheria related genes(16SrRNA,dtxR,toxA,toxB)were detected by PCR method.Results A Corynebacterium diphtheria was isolated from the patient′s sputum,and the results of biochemical detection,mass spectrometry and 16SrRNA sequencing were consistent with the characteristics of Corynebacterium diphtheria.The dtxR,toxA and toxB gene of the strains were negative.At the same time,the identification result of Corynebacterium striatum by biochemical reaction and mass spectrometry was consistent with Corynebacterium striatum.Conclusion The case with C.diphtheria is not carrier of toxin gene,is a carrier of C.diphtheria with no toxin.C.diphtheriae and Corynebacterium striatum can be quickly identified and distinguished by MALDI-TOF MS method and is helpful to quick clinical diagnosis.
