老年T2DM病人外周血CD8^(+)CD25^(+)调节性T细胞比率变化及其与胰岛素抵抗的关系研究

摘　　要：目的探讨老年T2DM病人外周血CD8^(+)CD25^(+)调节性T细胞(Treg)比率变化及其与胰岛素抵抗的关系。方法选取2019年4月至2020年9月本院收治的42例老年T2DM病人及40例在本院进行体检的健康者,分别记为研究组、对照组。对比2组研究对象的一般资料、生化指标、CD8^(+)CD25^(+)Treg比率及胰岛素抵抗指数(HOMA-IR)。采用Pearson相关系数描述老年T2DM病人CD8^(+)CD25^(+)Treg比率变化与HOMA-IR的关系。结果研究组BMI、TC、LDL-C、FPG、餐后2 h血糖(2hPG)、HbA1c及空腹胰岛素(FINS)水平均高于对照组(P<0.05)。研究组HOMA-IR高于对照组(P<0.01),CD8^(+)CD25^(+)Treg比率低于对照组(P<0.01)。CD8^(+)CD25^(+)Treg比率与HOMA-IR、FINS均呈负相关(r=-0.506、-0.382,P<0.05),与HbA1c、2hPG、FPG、LDL-C及TC均无相关性(P>0.05)。结论相比健康者,老年T2DM病人CD8^(+)CD25^(+)Treg比率异常偏低,其可能与老年T2DM病人胰岛素抵抗相关。Objective To investigate the change in the ratio of CD8^(+)CD25^(+)regulatory T cells(Treg)in peripheral blood in the elderly patients with type 2 diabetes mellitus(T2DM),and to investigate the relationship with insulin resistance.Methods From April 2019 to September 2020,42 elderly patients with T2DM and 40 healthy elderly who underwent physical examination in our hospital were selected and divided into the study group and the control group,respectively.The general data,biochemical indicators,CD8^(+)CD25^(+)Treg ratio and insulin resistance index(HOMA-IR)of the two groups were compared.Pearson correlation analysis was used to explore the relationship between the change of CD8^(+)CD25^(+)Treg ratio and HOMA-IR in the elderly patients with T2DM.Results The levels of body mass index(BMI),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),fasting plasma glucose(FPG),postprandial 2 h plasma glucose(2hPG),glycosylated hemoglobin(HbA1c)and fasting insulin(FINS)in the study group were higher than those in the control group(P<0.05).The level of HOMA-IR in the study group was higher than that in the control group(P<0.01),and the ratio of CD8^(+)CD25^(+)Treg was lower than that in the control group(P<0.01).CD8^(+)CD25^(+)Treg ratio was negatively correlated with HOMA-IR and FINS(r=-0.506,-0.382,P<0.05).Conclusions Compared with healthy elderly,the ratio of CD8^(+)CD25^(+)Treg in elderly T2DM patients is abnormally lower,which may be related to insulin resistance in elderly T2DM patients.
