摘　　要：目的探索心理健康因素对人类免疫缺陷病毒(human immunodeficiency virus, HIV)感染者抗病毒治疗依从性的影响,为后期制定提高抗病毒治疗依从性的策略提供科学依据。方法采用方便抽样的方法抽取调查对象,采用问卷调查法对调查对象的社会支持系统、艾滋病压力、焦虑和抑郁情绪以及应对方式进行心理健康测试,并探索其对抗病毒治疗依从性的影响。结果本研究共调查410例正在接受抗病毒治疗的HIV感染者,抗病毒治疗依从性好者占12.68%,依从性差者占87.32%。多因素分析结果显示,感染者社会支持越好,其抗病毒治疗依从性越好(OR=1.54,95%CI:1.34~1.78,P<0.01);艾滋病压力(OR=0.53,95%CI:0.34~0.81,P<0.01)和消极应对得分(OR=0.85,95%CI:0.76~0.96,P<0.01)与抗病毒治疗接受性呈负相关。结论 HIV感染者心理健康水平较低,且心理健康状况对HIV感染者抗病毒治疗依从性有较大影响,建议在抗病毒治疗随访咨询中,加强对HIV感染者的心理支持,提高其治疗依从性,从而改善其健康状况。Objective To elucidate the impact of mental health on the adherence to antiretroviral therapy(ART) among people living with HIV in Yunnan province, and to provide a scientific evidence for formulating strategies to improve the adherence to ART. Methods A convenient sampling method was used to recruit study participants and a questionnaire survey was conducted to assess their mental health status by analyzing the social support system, AIDS stress, anxiety and depression as well as disease coping style of the subjects, in order to explore the impact of mental health factors on the adherence to ART. Results A total of 410 PLWHA were recruited.Only 12.68% participants had decent compliance to ART.Multivariate analysis showed that AIDS stress(OR=0.53, 95%CI: 0.34-0.81, P<0.01)and score of passive attitude(OR=0.85, 95%CI:0.76-0.96, P<0.01)were negatively correlated with the compliance to ART, while social support was positively correlated with the treatment compliance(OR=1.54, 95%CI: 1.34-1.78, P<0.01). Conclusions The mental health of PLWHA has a great impact on the adherence to ART.It is suggested that routine follow-up counseling should provide psychological support for PLWHA to improve their mental health level, so as to enhance their adherence to ART.
