中西医结合快速康复外科在老年混合痔患者围手术期的应用

摘　　要：目的 探讨中西医结合快速康复外科在老年混合痔患者围手术期中的应用价值。方法 将80例老年混合痔患者随机分为观察组和对照组,每组各40例。观察组予中西医结合快速康复外科处理方案,对照组予常规处理方案。比较两组入院时及术前焦虑自评量表(SAS)评分及抑郁自评量表(SDS)评分;术后并发症(术后当日尿潴留评分及术后第1、3、7 d切口疼痛、水肿、出血评分);住院时间及切口愈合时间。结果观察组术前焦虑自评量表(SAS)评分及抑郁自评量表(SDS)评分低于对照组,差异有统计学意义(P <0.05);观察组术后当日尿潴留评分低于对照组,观察组术后第1、3、7 d切口疼痛、水肿及出血评分均低于对照组,差异有统计学意义(P <0.05);观察组住院时间及切口愈合时间短于对照组,差异有统计学意义(P <0.05)。结论 在老年混合痔患者围手术期应用中西医结合快速康复外科理念可有效改善患者围手术期焦虑、抑郁状态,加速术后恢复,减少并发症的发生,可在临床推广应用。Objective To evaluate the application value of integrated traditional Chinese and Western medicine fast-track surgery in elderly patients with mixed hemorrhoids during the perioperative period. Methods A total of 80 elderly patients with mixed hemorrhoids were randomly divided into observation group and control group(n = 40). The observation group was treated with integrated traditional Chinese and Western medicine fast-track surgery,while the control group was given conventional treatment. The two groups were compared in terms of Self-Rating Anxiety Scale(SAS)and Self-Rating Depression Scale(SDS)scores on admission and before operation,and postoperative complications(the urinary retention score on the 1st day after operation,and the scores of incision pain,edema,and bleeding on the 1st,3rd,and 7th days after operation),length of hospital stay,and incision healing time. Results Compared with the control group,the observation group had significantly lower preoperative SAS score and SDS score,urinary retention score on the 1st day after operation,and scores of incision pain,edema,and bleeding on the 1st,3rd,and 7th days after operation,as well as significantly shorter length of hospital stay and incision healing time(all P < 0.05). Conclusion The application of integrated traditional Chinese and Western medicine fast-track surgery in elderly patients with mixed hemorrhoids during the perioperative period can effectively relieve anxiety and depression,accelerate postoperative recovery,and reduce the incidence of complications,which holds promise for clinical application.
