Reflections on the construction of fever clinics in the post-epidemic era

摘  要:目的对南疆地区的发热门诊相关工作人员工作现状进行分析,为发热门诊更为高效地运转提供参考。方法采用自编问卷,通过问卷星对南疆地区医疗机构工作人员进行问卷调查,涉及的内容包括:发热门诊功能设置、人员配备、绩效管理、职称考评以及职业发展预期。结果对于发热门诊功能设置,绝大多数人(95.36%)认为设置发热门诊是必要的,并且理应成为单独一个科室(64.29%),但在其所行使的职能上存在分歧:36.25%认为该门诊仅进行新冠肺炎疾病的筛查,新冠核酸阴性即转诊;32.68%认为必须对所有发热病人全程诊治,30%认为也可以进行普通发热性疾病的初步诊疗。在人员配备上,半数发热门诊全体人员固定(49.46%)且认为应该固定(59.29%);仅有10.54%的参与人员所在医院发热门诊的医护人员完全不固定,并且只有20.54%认为不应该固定发热门诊医护人员。绩效管理上,对目前发热门诊的绩效感觉满意为79.29%,不满意的仅占16.79%,一半以上的人的绩效是独立核算(53.04%)并且认为应该独立核算(71.79%)。对于职称考评,无论是医生还是护士,绝大多数都认为应该参与职称考评,分别为88.89%和83.46%。而在职业发展预期问题上,仅有38.16%的医生、42.11%的护士愿意长期留在发热门诊;主要原因是认为会影响自身的职业发展(医生占65.70%,护士占51.50%)。结论根据目前疫情形势,建设发热门诊成为共识。南疆地区发热门诊人员学历以本科和专科为主,并且人员较为固定,在面对复杂发热病情时难以及时有效地做出正确判断,导致当地发热门诊只能进行单一的新冠肺炎筛查,有必要对工作人员进行系统培训,提高相关专业知识与技术。虽然发热门诊绩效达到了医生和护士们心中的预期,但大多数人们仍不愿意长期在发热门诊工作,如何改善这一现状需要深入调研与思考。Objective This paper analyzes the working status of the relevant staff of fever clinics in southern Xinjiang,and provides a reference for the more efficient operation of fever clinics.Methods A self-made questionnaire was used to conduct a questionnaire survey on the staff of medical institutions in southern Xinjiang through Questionnaire Star.The contents involved include:fever clinic function setting,staffing,performance management,professional title evaluation and career development expectations.Results For the function setting of the fever clinic,the vast majority of people(95.36%)believe that the setting of the fever clinic is necessary and should be a separate department(64.29%),but there are differences in the functions it performs:36.25% believe that the clinic is only a single department.Screening for new coronary pneumonia disease,the new crown nucleic acid is negative,that is,referral;32.68% believe that all febrile patients must be diagnosed and treated throughout the process,and 30% believe that preliminary diagnosis and treatment of common febrile diseases can also be carried out.In terms of staffing,half of the fever clinic staff are fixed(49.46%)and believe that it should be fixed(59.29%);only 10.54% of the medical staff in the fever clinic of the hospital where the participants are located are not fixed at all,and only 20.54% believe that it should not be fixed.Fever clinic medical staff.In terms of performance management,79.29% were satisfied with the performance of the current fever clinics,and only 16.79% were dissatisfied.More than half of the people’s performance was independently accounted for(53.04%)and thought it should be accounted for independently(71.79%).For professional title evaluation,the vast majority of doctors and nurses believe that they should participate in professional title evaluation,accounting for 88.89% and 83.46% respectively.On the issue of career development expectations,only 38.16% of doctors and 42.11% of nurses were willing to stay in the fever clinic for a long
