摘　　要：目的:设计一种用于永久性起搏器植入手术可调式头架,并评价其临床应用效果。方法:选取医院心导管室收治的100例植入永久性起搏器患者,采用随机数表法将其分为观察组和对照组,每组50例。植入手术中对照组采用简易支架;观察组采用永久性起搏器植入手术可调式头架。对比两组患者术中发生憋闷、幽闭恐惧、术中患者头侧温湿度变化、患者使用舒适度及术者临床体验满意度。结果:术中观察组患者发生憋闷、幽闭恐惧等症状少于对照组患者,差异有统计学意义(χ^(2)=50.174,χ^(2)=4.891;P<0.05);观察组患者舒适度中呼吸受限、紧张或压迫感和闷热感均高于对照组患者,差异有统计学意义(t=-35.307,t=-33.417,t=-34.385;P<0.05);对照组患者头侧温度和相对湿度均高于观察组,差异有统计学意义(t=-74.143,t=-126.652;P<0.05);手术医生对观察组可调式头架使用的便捷性、高效性及实用性得分高于对照组的简易支架方法,差异有统计学意义(t=-33.209,t=-35.492,t=-33.330;P<0.05)。结论:永久性起搏器植入手术可调式头架的设计合理、科学,可支撑永久性起搏器手术所用的无菌单,且不影响术者操作和血管造影机C臂的正常运转,并可增加患者术中舒适度,促进永久性起搏器手术的顺利进行。Objective:To design an adjustable headframe for implanting operation of permanent pacemaker and evaluate the clinical application effect of that.Methods:100 patients who were implanted by permanent pacemaker in the cardiac catheterization room of hospital were randomly divided into observation group and control group according to random number table,with 50 cases in each group.Simple stent was used in control group,and the observation group adopted an adjustable head frame for implanting operation of permanent pacemaker.The occurrences of feeling suffocation and claustrophobia,and the changes of temperature and humidity on the side of the head of intraoperative patients,the comfort level of patients in use and the satisfaction of clinical experience of them were compared between the two groups.Results:The symptoms of occurring suffocation and claustrophobia in observation group were significantly less than those in control group,the differences of them between two groups were statistically significant(χ^(2)=50.174,χ^(2)=4.891,P<0.05).The breathing obstruction,tension or constriction,and stuffiness in comfort degrees of observation group were significantly higher than those of control group(t=-35.307,t=-33.417,t=-34.385,P<0.05).The temperature on the side of head and relative humidity in control group were significantly higher than those in observation group(t=-74.143,t=-126.652,P<0.05).The scores of doctors for convenience,high efficiency and practicability of adjustable head frame of observation group were significantly higher than those of conventional method of control group(t=-33.209,t=-35.492,t=-33.330,P<0.05).Conclusion:The design of adjustable head frame for surgery of permanent pacemaker is reasonable and scientific,which can support the sterile coverlet in the surgery of permanent pacemaker,and it does not affect the operation of operators and the normal operation of C-arm of angiography machine.And it can increase the intraoperative comfort level of patients and promote the successful implementation o
