摘　　要：目的:探讨超声弹性成像参数在出血性卵巢囊肿与子宫内膜异位症鉴别诊断中的应用价值。方法:选取医院收治的96例盆腔肿物患者,根据手术病理诊断结果将子宫内膜异位症患者纳入内膜异位组(46例),将出血性卵巢囊肿患者纳入卵巢囊肿组(50例)。统计两组患者的年龄、平均病灶直径、体质量指数(BMI)及合并疾病等,比较两组患者超声弹性成像剪切波速度(SWV)值,以及弹性模量平均值、最大值和最小值;采用受试者工作特征(ROC)曲线分析超声弹性成像参数对出血性卵巢囊肿与子宫内膜异位症的鉴别诊断价值。结果:两组患者的年龄、平均病灶直径、BMI、高血压、糖尿病等临床资料比较均无统计学差异;内膜异位组SWV值,以及弹性模量平均值、最大值和最小值均高于卵巢囊肿组,差异有统计学意义(t=30.463,t=20.635,t=32.158,t=15.199;P<0.05)。超声弹性成像SWV值、弹性模量平均值、最大值和最小值对出血性卵巢囊肿诊断的ROC曲线下面积(AUC)分别为0.866、0.812、0.870和0.833。结论:超声弹性成像参数对出血性卵巢囊肿与子宫内膜异位症具有一定的鉴别诊断价值。Objective:To explore the application value of ultrasound elastography parameters in the differential diagnosis of hemorrhagic ovarian cysts and endometriosis.Methods:A total of 96 patients with pelvic masses who admitted to hospital were selected.According to the results of surgical and pathological diagnosis,46 patients with endometriosis were included in the endometriosis group and 50 patients with hemorrhagic ovarian cyst were included in the ovarian cyst group.The age,average lesion diameter,body mass index(BMI),comorbid disease,etc.of two groups were counted as statistic method,and ultrasound elastography shear wave velocity(SWV) values and the average value,maximum and minimum of elastic modulus of two groups were further compared.The receiver operating characteristic(ROC) curve was used to analyze the value of differential diagnosis of ultrasound elastography parameters for hemorrhagic ovarian cysts and endometriosis.Results:There were no significant differences in clinical data included age,average lesion diameter,BMI,hypertension and diabetes between the two groups.The SWV value,and the average value,maximum and minimum of elasticity modulus of endometriosis group were significantly higher than those of ovarian cyst group(t=30.463,t=20.635,t=32.158,t=15.199,P<0.05).The area under the curve(AUC) of ROC curve of SWV value of ultrasound elastography,average value,maximum and minimum of elastic modulus were 0.866,0.812,0.870 and 0.833 in diagnosing hemorrhagic ovarian cyst,respectively.Conclusion:Ultrasound elastography parameters have a certain of value in differential diagnosis for hemorrhagic ovarian cysts and endometriosis.
