摘  要:目的:研究磁共振成像(MRI)3D梯度回波Dixon技术对全肝脂肪肝的定量及治疗效果,评价该技术的可行性。方法:分析使用荷兰飞利浦3.0T高场强MRI3D梯度回波Dixon序列对32例脂肪肝患者治疗前后全肝快速扫描,所得数据经矩阵实验室(MATLAB)数据分析软件进行分析,自动生成全肝平均脂肪分数(FF),以肝穿刺活检为标准,分别与CT、常规MRI技术的准确率进行比较。结果:治疗前32例全肝脂肪肝最大FF为61%、最小FF为12%,治疗后最大FF为41%、最小FF为4%,11例经治疗后全肝平均FF降低20%以上,16例降低20%以下,5例恢复正常。磁共振3D梯度回波Dixon序列脂肪定量技术灵敏度为100%,特异度为100%。CT、常规MRI和3D梯度回波Dixon序列检查对全肝脂肪肝定量及治疗效果评价的准确率分别为59.375%(19/32)、62.500%(20/32)和93.750%(30/32);3D梯度回波Dixon技术与CT、常规MRI准确率比较,差异有统计学意义(χ^(2)=10.54,χ^(2)=9.14;P<0.05)。结论:MRI 3D梯度回波Dixon技术对全肝脂肪肝的定量及治疗效果的评价明显优于常规MRI和CT检查,可为临床脂肪肝的治疗效果提供重要的数据信息和依据。Objective:To explore the feasibility of magnetic resonance imaging(MRI)three dimensional(3D)gradient echo Dixon technique in quantification and therapeutic effect of fatty liver of whole liver.Methods:Retrospective analysis was used to analyze the fast scans of Philips 3.0T high field MRI 3D gradient echo Dixon sequence of Netherlands on 32 patients who underwent the treatment of fatty liver before and after treatment.The obtained data automatically generate average fat fraction(FF)of whole liver through the analysis of Matrix laboratory(MATLAB)data analysis software.The aspiration biopsy of liver was used as standard to compare with the accuracies of CT and conventional MRI technique.Results:The FF maximum and FF minimum of fatty liver of whole liver of 32 cases were 61%and 12%before treatment,while they were 41%and 4%after treatment,respectively.The decrease of average FF of whole liver of 11 cases was larger than 20%,and the decrease of that of 16 cases was less than 20%,and that of 5 cases recover normal.The sensitivity and specificity of MR 3D gradient echo Dixon sequence fat quantitative technique were 100%and 100%,respectively.The accuracy rates of CT,conventional MRI and 3D gradient echo Dixon sequence were 59.375%(19/32),62.500%(20/32)and 93.750%(30/32)in quantifying fatty liver of whole liver and evaluating therapeutic effect.There was significant difference in accuracy rate between 3D gradient echo Dixon technique and CT,and conventional MRI(χ^(2)=10.54,χ^(2)=9.14,P<0.05).Conclusion:The evaluation of MRI 3D gradient echo Dixon technique is superior to conventional MRI and CT examination in quantitation and therapeutic effect of fatty liver of whole liver.It can provide important data information and basis for the therapeutic effect of clinical fatty liver.
