摘　　要：目的探讨拉西地平联合西拉普利治疗对高血压病患者颈动脉超声指标、血管内皮功能及不良反应的影响。方法选取2017年10月—2020年10月本院收治的164例高血压病患者,根据治疗方案的不同分为观察组79例(予拉西地平联合西拉普利治疗)和对照组85例(单用西拉普利治疗)。比较2组治疗前后颈动脉超声、血管内皮功能指标差异及用药安全性;随访6个月,观察2组随访期间不良事件发生情况。结果治疗后,2组颈动脉内膜中层厚度(CIMT)、内皮素-1(ET-1)水平下降,肱动脉内皮依赖性舒张功能(FMD)、一氧化氮(NO)水平升高(P<0.01);治疗后,观察组CIMT、ET-1水平低于对照组,FMD、NO水平高于对照组(P<0.05)。2组不良反应总发生率比较差异无统计学意义(P>0.05)。观察组不良事件总发生率为8.86%显著低于对照组的20.00%(P<0.05)。结论拉西地平联合西拉普利治疗能明显改善高血压病患者颈动脉超声指标、血管内皮功能指标水平,且近期预后较好,安全、可行。Objective To investigate effects of Lacidipine combined with Cilazapril on carotid ultrasound indexes,vascular endothelial function and adverse reactions in treatment of patients with hypertension.Methods A total of 164 patients with hypertension admitted between October 2017 and October 2020 were divided into observation group(treated with Lacidipine combined with Cilazapril,n=79)and control group(treated with Cilazapril alone,n=85).Differences of carotid ultrasound indexes,vascular endothelial function indexes and drug safety before and after treatment were compared between two groups.All patients were followed up for 6 months to observe incidence rates of adverse events during the follow-up in two groups.Results After treatment,levels of carotid intima-media thickness(CIMT)and endothelin-1(ET-1)were significantly decreased,while levels of endothelium-dependent diastolic function(FMD)of brachial artery and nitric oxide(NO)were significantly increased(P<0.01).After treatment,in observation group,levels of CIMT and ET-1 were significantly lower,while levels of FMD and NO were significantly higher than those in control group(P<0.05).There was no significant difference in the total incidence rate of adverse reactions between two groups(P>0.05).The total incidence rate of adverse events in observation group(8.86%)was significantly lower than 20.00%in control group(P<0.05).Conclusion Lacidipine combined with Cilazapril may significantly improve levels of carotid ultrasound indexes and vascular endothelial function with good in treatment of patients with hypertension,and its short-term prognosis is good with good safety and feasibility.
