班布特罗联合噻托溴铵粉治疗慢性阻塞性肺气肿的效果及对肺功能的影响

摘　　要：目的研究班布特罗联合噻托溴铵粉治疗慢性阻塞性肺气肿的效果及对肺功能的影响。方法选取2019年11月至2020年10月本院接收的78例慢性阻塞性肺气肿患者作为研究对象,根据随机数字表法将其分为对照组与观察组,各39例。对照组给予噻托溴铵粉治疗,观察组给予班布特罗联合噻托溴铵粉治疗。比较两组的临床效果。结果治疗后,两组的气短、咳嗽咳痰、呼吸困难患者占比均低于治疗前,且观察组低于对照组(P<0.05)。观察组的气短、食欲不振、咳嗽咳痰、呼吸困难消失时间及住院时间均短于对照组(P<0.05)。治疗后,两组的PaO_(2)、SaO_(2)、FVC、FEV_(1)、PEF、FEV_(1)/FVC均高于治疗前,PaCO_(2)、CRP、IL-8、TNF-α、MMP-9、VAP-1水平均低于治疗前,且观察组优于对照组(P<0.05)。观察组的治疗总有效率高于对照组(P<0.05)。两组的不良反应总发生率比较,差异无统计学意义(P>0.05)。治疗后,两组的6MWD均长于治疗前,SGRQ评分均低于治疗前,且观察组优于对照组(P<0.05)。结论班布特罗联合噻托溴铵粉治疗慢性阻塞性肺气肿的效果显著,可促进患者临床症状、肺功能指标及血气指标有效改善,提高其生活质量,且安全性高。Objective To study the effect of bambuterol combined with tiotropium bromide powder in the treatment of chronic obstructive emphysema and its influence on lung function.Methods A total of 78 patients with chronic obstructive emphysema received in our hospital from November 2019 to October 2020 were selected as the research objects.The patients were divided into control group and observation group according to random number table method,with 39 cases in each group.The control group was treated with tiotropium bromide powder,and the observation group was treated with bambuterol combined with tiotropium bromide powder.The clinical effect of the two groups was compared.Results After treatment,the proportions of patients of shortness of breath,cough and expectoration and dyspnea in the two groups were lower than those before treatment,and those in the observation group were lower than the control group(P<0.05).The disappearance time of shortness of breath,loss of appetite,cough and expectoration,dyspnea and length of stay in the observation group were shorter than those in the control group(P<0.05).After treatment,PaO_(2),SaO_(2),FVC,FEV_(1),PEF and FEV_(1)/FVC in the two groups were higher than those before treatment,PaCO_(2),CRP,IL-8,TNF-α,MMP-9,VAP-1 levels were lower than those before treatment,and those in the observation group were better than the control group(P<0.05).The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).After treatment,the 6MWD in the two groups was longer than that before treatment,the SGRQ score was lower than that before treatment,and those in the observation group were better than the control group(P<0.05).Conclusion Bambuterol combined with tiotropium bromide powder in the treatment of chronic obstructive emphysema has a significant effect,it can promote the improvement of clinical symptoms,lung function indexes and bloo
