外周血CD4^(+)/CD8^(+)与脓毒症患者肠道菌群的关联性

摘　　要：目的分析外周血CD4^(+)/CD8^(+)与脓毒症患者肠道菌群的关联性。方法选取2019年10月至2021年4月濮阳市人民医院收治的82例脓毒症患者,纳入观察组,并同期选取69例健康体检者作为健康对照组。采集两组血液标本、粪便标本,测定CD3^(+)、CD4^(+)、CD8^(+)水平,计算CD4^(+)/CD8^(+),以及双歧杆菌、乳酸杆菌、大肠杆菌、肠球菌计数。比较两组CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)、双歧杆菌、乳酸杆菌、大肠杆菌、肠球菌计数。分析不同菌群紊乱程度脓毒症患者CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)水平,以探讨其与脓毒症患者肠道菌群的关联性。结果观察组外周血CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平较健康对照组低,CD8^(+)水平较健康对照组高(P<0.05);观察组乳酸杆菌、双歧杆菌计数较健康对照组低,大肠杆菌、肠球菌计数较健康对照组高(P<0.05);不同菌群紊乱程度脓毒症患者外周血CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)水平比较,差异有统计学意义(P<0.05);Ⅰ度菌群紊乱患者外周血CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平最高,CD8^(+)水平最低(P<0.05)。脓毒症患者外周血CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平均与双歧杆菌、乳酸杆菌计数呈正相关,与大肠杆菌、肠球菌计数呈负相关(P<0.05);CD8^(+)水平与双歧杆菌、乳酸杆菌计数呈负相关,与大肠杆菌、肠球菌计数呈正相关(P<0.05)。结论脓毒症患者肠道菌群、外周血CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)水平均存在异常表达的情况,且外周血CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)表达水平与肠道菌群存在相关性。Objective To analyze the relationship between peripheral blood CD4^(+)/CD8^(+) and intestinal flora of patients with sepsis.Methods A total of 82 patients with sepsis in Puyang People’s Hospital from October 2019 to April 2021 were selected as observation group,and 69 healthy physical examination subjects during the same period were selected as control group.Blood and stool samples of the two groups were collected.The levels of CD4^(+) and CD8^(+) were measured.CD4^(+)/CD8^(+) was calculated.The number of Bifidobacterium,Lactobacillus,Escherichia coli and Enterococcus were counted.The counts of CD4^(+),CD8^(+),CD4^(+)/CD8^(+),Bifidobacterium,Lactobacillus,Escherichia coli and Enterococcus were compared between the two groups.The levels of CD4^(+),CD8^(+) and CD4^(+)/CD8^(+) in sepsis patients with different degrees of flora disturbance were analyzed,and to explore the correlation between them and intestinal flora in sepsis patients.Results The levels of CD4^(+) and CD4^(+)/CD8^(+) in observation group were lower than those in control group,and the level of CD8^(+) in observation group was higher than that in control group(P<0.05).The counts of Lactobacillus and Bifidobacterium in observation group were lower than those in control group,and the counts of Escherichia coli and Enterococcus were higher than those in control group(P<0.05).The levels of CD4^(+),CD8^(+),CD4^(+)/CD8^(+) of sepsis patients with different levels of flora disorders were compared,and the differences were statistically different(P<0.05).The levels of CD4^(+),CD4^(+)/CD8^(+) of patients with first-degree flora disorders were the highest,and the level of CD8^(+)was the lowest(P<0.05).The levels of CD4^(+) and CD4^(+)/CD8^(+) were positively correlated with the counts of Bifidobacterium and Lactobacillus,and negatively correlated with the counts of Escherichia coli and Enterococcus(P<0.05).The level of CD8^(+) was negatively correlated with the counts of Bifidobacterium and Lactobacillus,and it was negatively correlated with the counts of Esch
