摘  要:目的比较免疫组织化学法(Immunohistochemistry,IHC)与聚合酶链反应法(Polymerase chain reaction,PCR)检测梅毒患者皮损组织蜡块中梅毒螺旋体(Treponema pallidum,TP)的敏感性。方法对广西医科大学第一附属医院皮肤科26名诊断为梅毒的患者皮损组织蜡块分别进行IHC和PCR检测。结果26份样本均显示IHC阳性,阳性率为100%;有7例Tp47-PCR阳性,9例poIA-PCR阳性,阳性率分别为27%和35%,其中poIA-PCR阳性样品中包含了Tp47-PCR阳性样品。结论IHC对梅毒患者皮损组织蜡块中TP的检测敏感性高于PCR,对于临床出现多样化皮疹,高度怀疑梅毒却出现诊断困难时,IHC可作为一种针对皮损组织蜡块的准确、敏感的特异性检测方式,帮助临床诊断。Objective To compare the sensitivity of immunohistochemistry(IHC)and polymerase chain reaction(PCR)to TP in paraffin-embedded skin biopsies from syphilis patients.Methods IHC and PCR were performed on paraffin-embedded skin biopsies of 26 patients diagnosed with syphilis in the Dermatology Department of the First Affiliated Hospital of Guangxi Medical University.Results All the 26 samples showed positive IHC,with positive rate of 100%.7 cases were positive for TP47-PCR and 9 cases were positive for poIA-PCR,with positive rate of 27% and 35%,respectively.The positive samples of poIA-PCR included the positive samples of TP47-PCR.Conclusion IHC is more sensitive to the detection of TP in paraffin tissue of syphilis patients than PCR.For patients with diverse clinical rash lesions and highly suspected syphilis,IHC can be regarded as an accurate and sensitive specific detection method to help clinical diagnosis.
