应用措施集合管理预防口腔颌面皮瓣移植术后患者血管危象的效果研究

摘　　要：目的:探讨应用措施集合管理预防口腔颌面皮瓣移植术后患者血管危象的效果。方法:前瞻性研究。将2018年1月至2020年12月在我院接受口腔颌面皮瓣移植术治疗的100例患者按照随机数字表法分为2组,对照组接受常规干预,研究组加用措施集合管理干预,比较2组患者手术治疗情况、心理状况、疼痛情况、血管危象等并发症情况。结果:2组患者手术时间、术中出血量比较差异无统计学意义(P>0.05),研究组供区创面愈合时间、受区创面愈合时间、住院治疗时间均短于对照组(P<0.05);2组患者手术前SAS评分、SDS评分、VAS评分比较差异无统计学意义(P>0.05),术后研究组SAS评分、SDS评分、VAS评分均低于对照组(P<0.05);研究组血管危象等并发症发生率低于对照组(P<0.05)。结论:口腔颌面皮瓣移植术患者接受措施集合管理模式干预,可有效缩短供区、受区创面愈合时间与住院治疗时间,缓解负性情绪,减轻术后疼痛的同时,减少术后血管危象等并发症的发生。Objective:To investigate the preventive effect of combined management on vascular crisis in patients after oral and maxillofacial skin flap transplantation.Methods:A total of 100 patients accepted oral and maxillofacial skin flap transplantation therapy in our hospital from Jan 2018 to Dec 2020 were divided into two groups using random number table method.The control group accepted routine intervention,and the study group accepted combined management.The operative time,intraoperative blood loss,psychological status,pain,and the incidence of vascular crisis were compared.Results:There were no significant differences in operative time and intraoperative blood loss between the two groups(P>0.05).The healing time of donor area,healing time of recipient area and hospitalization time in the study group were shorter than those in the control group(P<0.05).There were no significant differences in preoperative psychological status and pain evaluation between the two groups(P>0.05).The scores of SAS,SDS and VAS in the study group were lower than those in the control group after intervention(P<0.05),and the incidence of vascular crisis in the study group was lower than that in the control group(P<0.05).Conclusion:Application of combined management in patients with oral and maxillofacial skin flap transplantation can effectively shorten wound healing time and hospitalization time,relieve negative moods,alleviate postoperative pain,reduce the incidence of vascular crisis.
