改良头低足高位在腹腔镜膀胱癌根治术联合回肠代膀胱术患者中的应用

摘　　要：目的:探讨改良头低足高位在腹腔镜膀胱癌根治术联合回肠代膀胱术患者中的应用效果。方法:将2019年3月1日~2020年12月31日收治的76例腹腔镜膀胱癌根治术联合回肠代膀胱术患者随机分为观察组和对照组各38例;对照组采用15°头低足高位,观察组采用改良头低足高位,角度为45°;比较两组手术指标及术后恢复情况、压力性损伤风险及并发症发生情况。结果:观察组手术时间、术后胃肠功能恢复时间均短于对照组(P<0.01);观察组术中出血量少于对照组(P<0.01);两组均未出现压力损伤极高风险病例,观察组压力损伤发生风险率低于对照组(P<0.05);观察组总并发症发生率低于对照组(P<0.05)。结论:改良头低足高位在腹腔镜膀胱癌根治术联合回肠代膀胱术中的临床应用效果好,可缩短手术时间和住院时间,降低并发症发生率及压力性损伤风险,改善患者预后。Objective:To explore the application effect of head low foot high position in patients with laparoscopic radical cystectomy plus ileal bladder replacement.Methods:A total of 76 patients with laparoscopic radical cystectomy and ileal bladder replacement admitted during March 1,2019 to December 31,2020 were randomly divided into the observation group and the control group,38 patients in each group.The control group adopted 15°head low foot high,and the observation group adopted improved head low foot high,the angle was 45 degrees.The operation indicators,the postoperative recovery,the risk assessment of stress injury and complications were compared between the two groups.Results:The operation time and the recovery time of postoperative gastrointestinal time in the observation group was shorter than that in the control group(P<0.01),the intraoperative blood loss in the observation group was less than that in the control group(P<0.01);there was no high-risk case of pressure injury in the two groups,and the observation group had no significant difference The risk rate of pressure injury risk assessment in the observation group was lower than that in the control group(P<0.05);and the total complication rate in the observation group was lower than that in the control group(P<0.05).Conclusion:The modified head low foot high position in laparoscopic radical cystectomy plus ileal bladder replacement has good clinical effect,can shorten the operation time,reduce the length of hospital stay,reduce the risk of complications and pressure injury,and improve the prognosis of patients.
