摘 要:目的探讨分析磁敏感加权成像在帕金森病(PD)诊断及病情严重程度评估的价值。方法选取广东省信宜市人民医院2019年1月至2020年6月收治的40例帕金森患者作为实验组,选取同时期健康体检中心的健康人群40例作为对照组,均给予磁敏感加权成像检查,比较两组红核、黑质网状带、黑质致密带、壳核、尾状核和苍白球相位值,不同Hoehn-Yahr分级帕金森患者各核团相位值。结果两组红核、黑质网状带相位值比较,差异无统计学意义(P>0.05),实验组黑质致密带、壳核、苍白球相位值低于对照组,尾状核相位值高于对照组(P<0.05),PD患者Hoehn-Yahr分级与黑质网状带、黑质致密带、壳核、苍白球相位值相关,且黑质网状带、黑质致密带、壳核、苍白球相位值与Hoehn-Yahr分级呈现负相关(P<0.05),而PD患者红核、尾状核相位值与Hoehn-Yahr分级无相关性(P>0.05)。结论磁敏感加权成像(SWI)应用于PD患者的诊断和病情评估中是有效的,根据脑部各核团相位值的变化可判断黑质、壳核等铁沉积含量,进而分析患者疾病的严重程度,为疾病治疗提供依据,在临床上具有广泛的应用前景。【Objective】To explore the value of susceptibility weighted imaging(SWI)in the diagnosis and severity assessment of Parkinson’s disease(PD).【Methods】Forty Parkinson’s patients who were clinically admitted from January 2019 to June 2020 were selected as the experimental group,and forty healthy people in the health check-up center during the same period were selected as the control group;they were all given SWI examinations.The phase values of the red nucleus,substantia nigra reticular zone,substantia nigra compact zone,putamen,caudate nucleus,and globus pallidus were compared between the two groups,and the phase values of each nucleus of PD patients with different Hoehn-Yahr grades were compared.【Results】There was no significant difference in the phase values of the red nucleus and substantia nigra reticulum zone between the two groups(P>0.05).The phase values of substantia nigra dense zone,putamen,the globus pallidus of the experimental group were significantly lower,and the phase value of the caudate nucleus was significantly higher in the experimental group than those in the control group(P<0.05).The HoehnYahr classification of PD patients was significantly related to the phase values of the substantia nigra reticular zone,substantia dense zone,putamen,and globus pallidus,and the phase values of the substantia nigra reticular zone,substantia nigra compact zone,putamen,and globus pallidus were negatively correlated with the Hoehn-Yahr classification(P<0.05),while the red nucleus and caudate nucleus phase values of PD patients were not significantly related to the Hoehn-Yahr classification(P>0.05).【Conclusion】Magnetic susceptibility weighted imaging(SWI)is effective in the diagnosis and disease assessment of PD patients.Through the changes in the phase value of each nucleus in the brain,we can judge the content of iron deposits in the substantia nigra,putamen,etc.,and then analyze the severity of the patient’s disease,which can provide a basis for disease treatment and have a wide range of cl
