彩色高频超声对良性阵发性位置性眩晕的诊断价值探析

摘　　要：目的探讨彩色高频超声对良性阵发性位置性眩晕(BPPV)的诊断价值。方法回顾性分析2019年1月至2019年12月漯河市中心医院收治的85例BPPV患者临床资料,根据眩晕残障量表(DHI)评分分为轻度组63例(DHI评分<60分)和中重度组22例(DHI评分≥60分)。两组患者均使用彩色高频超声检查,记录比较两组椎动脉指标[管腔内径、阻力指数(RI)、收缩期峰值流速(PSV)、舒张末期血液流速(EDV)]。采用Pearson相关性分析管腔内径、RI、PSV、EDV与DHI评分的相关性;采用受试者工作特征(ROC)曲线评估管腔内径、RI、PSV、EDV及联合诊断BPPV患者病情程度的诊断价值。结果中重度组管腔内径、PSV、EDV小于轻度组,而RI明显大于轻度组(P<0.05)。管腔内径、PSV、EDV与DHI评分呈负相关关系(P<0.05),RI与DHI评分呈正相关关系(P<0.05)。管腔内径、RI、PSV、EDV的ROC曲线的曲线下面积(AUC)分别为0.846、0.716、0.671、0.864;截断值分别为2.941 mm、0.590、50.00 cm/s、25.00 cm/s。联合诊断的ROC曲线AUC为0.968,敏感度为95.45%,特异度为92.06%,联合评估的AUC明显大于管腔内径、RI、PSV、EDV单独评估(P<0.05)。结论通过彩色高频超声可诊断出BPPV患者椎动脉管腔狭窄、血流速度下降,其为临床诊断BPPV患者病情严重程度提供依据。【Objective】To investigate the diagnostic value of color high frequency ultrasound in benign paroxysmal positional vertigo(BPPV).【Methods】The clinical data of 85 BPPV patients in our hospital were analyzed retrospectively.According to the scores of vertigo disability scale(DHI),the patients were divided into mild group(63 cases,DHI score<60),and medium and severe group(22 cases,DHI score≥60).The two groups were examined by color high frequency ultrasound,and the vertebral artery indexes[lumen diameter,resistance index(RI),peak systolic velocity(PSV),end diastolic blood flow rate(EDV)]were recorded and compared.Pearson correlation was used to analyze the correlation between lumen diameter,RI,PSV,EDV and DHI score.The working characteristics(ROC)curve of subjects were used to evaluate the diagnostic value of lumen diameter,RI,PSV,EDV and the degree of disease in BPPV patients.【Results】The lumen diameter,PSV and EDV of the moderate and severe group were significantly smaller than those in the mild group(P<0.05),while RI was significantly larger than that in the mild group(P<0.05).The results showed that the inner diameter of lumen,PSV,EDV were negatively correlated with DHI scores,and RI was positively correlated with DHI scores(P<0.05).The AUC of ROC curves of the lumen inner diameter,RI,PSV and EDV were 0.846,0.716,0.671,0.864 respectively;the cut-off values were 2.941 mm,0.590,50.00 cm/s and 25.00 cm/s respectively.The AUC of ROC curve of combination diagnosis was 0.968,sensitivity 95.45%,specificity 92.06%,and AUC was significantly larger than that of internal diameter,RI,PSV and EDV(P<0.05).【Conclusion】The stenosis of vertebral artery lumen and the decrease of blood flow rate can be diagnosed by color high frequency ultrasound,which can provide the basis for clinical diagnosis of the severity of BPPV.
