针刺治疗青光眼视神经萎缩的研究进展

摘　　要：青光眼是世界排名第二位的致盲性眼病,视神经萎缩是造成青光眼致盲的主要原因。本文通过回顾相关研究发现:针刺治疗青光眼视神经萎缩的机理主要是对眼压调节、改善网膜血液循环、减轻一氧化氮和谷氨酸对视网膜神经节细胞毒性作用、对视网膜超微结构的保护、提高机体免疫功能。针刺选穴多眼部近端取穴结合远端辩证选穴,睛明、太阳、风池为取穴最多的穴位;辨证分型上青光眼视神经萎缩以肝郁气滞型居多。其窍明穴、眼三针(睛明、承泣、上明)、韦氏三联九针、新明二穴均有良好疗效。此外,电针、梅花针叩刺、耳穴等均显示一定疗效,临床可配合使用。但据目前公开文献,针刺对青光眼视神经萎缩文献数量较少、缺乏深入机制研究,临床上选穴标准不一、样本量小、缺乏远期预后追踪等,需进一步改进。Glaucoma is the second blinding eye disease in the world.Optic nerve atrophy is the main cause of glaucoma blindness.By reviewing the relevant studies,it is found that the main mechanisms of acupuncture are:regulating intraocular pressure,improving omental blood circulation,reducing the toxic effect of nitric oxide and glutamate on retinal ganglion cells,protecting retinal ultrastructure and improving immune function.Most of the acupuncture points are selected from the near end of the eye combined with the dialectical selection of the far end.Jingming,Taiyang and Fengchi are the most selected points;The majority of glaucomatous optic nerve atrophy is liver stagnation and qi stagnation in syndrome differentiation.Its Qiaoming point,three eye acupuncture(Jingming,Chengqi and Shangming),Webster triple nine acupuncture and two Xinming points have good curative effects.In addition,electroacupuncture,plum blossom needle percussion and auricular points show certain curative effects,which can be used together in clinic.However,there are few literatures on acupuncture on glaucomatous optic atrophy,lack of in-depth mechanism research,different clinical point selection standards,small sample size and lack of long-term prognosis tracking,which need to be further improved.
