摘　　要：目的:调查当前河南地区慢性丙型肝炎(CHC)患者的丙型肝炎病毒(HCV)基因型分布、变化情况,并对HCV基因型流行特征的相关因素进行分析。方法:HCV基因分型采用Core和(或)NS5B测序进化树分型,单变量和多变量方法分析HCV基因型分布相关因素。结果:本研究共检测到7种基因型:1b(58.6%)、2a(15.7%)、6a(13.8%)、3a(7.5%)、3b(2.8%)、1a(0.8%)、2b(0.8%)。356例患者纳入统计分析,logistic回归模型显示,1b型与输血或血制品感染途径相关(OR=2.22,P=0.009),6a型与吸毒感染途径相关(OR=3.18,P=0.009);1b型多见于40岁以上患者(OR=1.02,P=0.023),而6a型多见于40岁及以下患者(OR=0.97,P=0.029)。结论:HCV基因型分布变化反映了传播途径的转变,现阶段河南地区HCV 1b型多见于40岁以上患者,6a型有增长趋势且多见于40岁及以下患者。Objective To investigate the distribution and change of hepatitis C virus(HCV)genotype in patients with chronic hepatitis C(CHC)in Henan province,and to analyze the factors related to the epidemic characteristics of HCV genotype.Methods HCV genotyping was performed by Core and/or NS5B sequencing phylogenetic tree,univariate and multivariate methods to analyze the factors related to HCV genotype distribution.Results Seven genotypes were detected in this study:1b(58.6%),2a(15.7%),6a(13.8%),3a(7.5%),3b(2.8%),1a(0.8%),2b(0.8%).356 patients were included in the statistical analysis.Logistic regression model showed that type 1b was associated with the infection route of blood transfusion or blood products(OR=2.22,P=0.009),and type 6a was associated with the infection route of drug use(OR=3.18,P=0.009).Type 1b was more common in patients over 40 years old(OR=1.02,P=0.023),while type 6a was more common in patients over 40 years old and below(OR=0.97,P=0.029).Conclusion The change of HCV genotype distribution reflects the shift of transmission route,and currently,HCV type 1b is mostly found in patients over 40 years old in Henan,while type 6a has an increasing trend and is mostly found in patients 40 years old and younger.
