米非司酮联合醋酸甲羟孕酮治疗异常子宫出血的效果及对子宫内膜厚度和性激素水平的影响

摘　　要：目的:研究用米非司酮联合醋酸甲羟孕酮治疗异常子宫出血(AUB)的效果及对患者子宫内膜厚度和性激素水平的影响。方法:选取广州番禺区新造医院收治的80例AUB患者作为试验对象。随机将其分为单药组和联合组。用醋酸甲羟孕酮对单药组患者进行治疗,用米非司酮联合醋酸甲羟孕酮对联合组患者进行治疗,然后比较两组患者的临床疗效、子宫内膜的厚度、血清性激素的水平、阴道出血停止的时间、月经失血图评分法(PBAC)的评分及月经间期出血的发生率。结果:联合组患者治疗的总有效率高于单药组患者,治疗3个月及6个月后其子宫内膜的厚度均低于单药组患者,P<0.05。治疗6个月后,联合组患者血清雌二醇(E2)、促卵泡生成激素(FSH)、促黄体生成激素(LH)和孕酮(P)的水平均低于单药组患者,P<0.05。用药后,两组患者阴道出血停止的时间及月经间期出血的发生率相比,P>0.05;联合组患者的PBAC评分低于单药组患者,P<0.05。结论:用米非司酮联合醋酸甲羟孕酮治疗AUB的效果显著,能有效减轻患者的临床症状,改善其子宫内膜的厚度和血清性激素的水平。Objective:To study the effect of mifepristone combined with medroxyprogesterone acetate in the treatment of abnormal uterine bleeding(AUB)and its effect on endometrial thickness and sex hormone levels.Methods:a total of 80 PATIENTS with AUB were selected as experimental subjects.They were randomly divided into monotherapy group and combination group.With medroxyprogesterone acetate treatment in patients with single drug groups,with mifepristone combined medroxyprogesterone acetate couplet to form patients for treatment,then compare the clinical ef ficacy of two groups of patient s,the thickness of the endometrium and serum levels of sex hormones,stop bleeding time,menstrual blood loss figure scoring method(PBAC)score and the incidence of bleeding between menstrual period.Results:The total effective rate in the combined group was higher than that in the single drug group,and the endometrial thickness was lower than that in the single drug group after 3 and 6 months of treatment,P<0.05.After 6 months of treatment,the serum levels of estradiol(E2),follicle-stimulating hormone(FSH),luteinizing hormone(LH)and progesterone(P)in combined group were lower than those in single drug group,P<0.05.After medication,the time of vaginal bleeding stop and the incidence of bleeding between the two groups were compared,P>0.05;PBAC score in combination group was lower than that in single drug group,P<0.05.Conclusion:Mifepristone combined with medroxyprogesterone acetate has a significant effect on the treatment of AUB,which can effectively reduce the clinical symptoms of patients,improve the endometrial thickness and serum sex ho rmone levels.
