摘 要:目的研究人类免疫缺陷病毒(HIV)感染者外周血miR-21、miR-23a-3p表达变化,并分析两者对机会性感染的预测价值。方法选取2020年5月至2021年5月于唐河县疾病预防控制中心首次确诊的HIV感染者134例作为HIV感染组,并选取同期于我院行健康体检的80名健康志愿者作为健康组。在2组研究对象入组后每个月监测1次外周血miR-21、miR-23a-3p表达情况,共连续监测4次,对比分析miR-21、miR-23a-3p在HIV感染组、健康组中的表达差异。在对HIV感染组患者行连续4次的监测后随访30 d,30 d内是否出现机会性感染分为非机会性感染组、机会性感染组2个亚组。对比分析2组外周血miR-21、miR-23a-3p表达量,并分析两者对HIV感染者机会性感染的预测价值。结果HIV感染组在4次监测中外周血mi R-21、miR-23a-3p表达量均高于健康组,差异有统计学意义(P<0.05)。机会性感染组在外周血mi R-21、miR-23a-3p表达量均高于非机会性感染组,差异有统计学意义(P<0.05)。miR-21、mi R-23a-3p与HIV感染者机会性感染呈正相关(r=0.832,P<0.05;r=0.865,P<0.05)。与miR-21、mi R-23a-3p单项预测相比,两项联合预测HIV感染者机会性感染的价值较高(P<0.05)。结论miR-21、miR-23a-3p在HIV感染者外周血中表达升高,其与患者机会性感染相关,可用于机会性感染的预测。Objective To study the expression of microRNA-21(miR-21) and micro-RNA-23 a-3 p(miR-23 a-3 p) in the peripheral blood of patients infect with human immuno-deficiency virus(HIV) and to analyze the predictive value of these biomarkers in opportunistic infections. Methods A total of 134 HIV-infected patients who were first diagnosed in our hospital from May 2020 to May 2021 were selected for analysis(HIV infection group). Data were also analyzed from 80 healthy volunteers who underwent physical examinations during the same period. The expression of miR-21 and miR-23 a-3 p in the per-ipheral blood was monitored once a month in the 2 groups of patients after enrollment and then continuously on 4 separate occasions. In the HIV-infected group, the expression of miR-21 and miR-23 a-3 p was monitored on 4 consecutive occasions and followed up for30 days. Depending on the occurrence of opportunistic infections at 30 days, the groups were divided into non-opportunistic infection and an opportunistic infection groups. The expres-sion of miR-21 and miR-23 a-3 p was measured in the peripheral blood of the two groups and the predictive value for opportunistic infection in HIV-infected patients was analyzed. Results The expressions of miR-21 and mi R-23 a-3 p in the peripheral blood of HIV infected patients were significantly higher compareood to patients in the healthy control group(P<0.05).Also, the expression of miR-21 and miR-23 a-3 p in the opportunistic in-fection group were significantly higher compared to the non-opportunistic infection group(P <0.05). The expres-sion of miR-21 and miR-23 a-3 p was positively correlated with opportunistic infections in HIV infected persons(r=0.832, P<0.05;r=0.865, P<0.05).The combined predictive power of miR-21 and miR-23 a-3 p for oppor-tunistic in-fections in HIV infected patients was higher(P<0.05) compared to miR-21 and miR-23 a-3 p alone.Conclusion The expression of miR-21 and mir-23 a-3 p is increased in the peripheral blood of HIV infected patients and the levels of these biomarke
