摘  要:目的探讨皮椎体形成术中不同黏度骨水泥对骨质疏松椎体压缩性骨折(OVCF)患者椎体形态和骨水泥渗漏情况的影响。方法回顾性分析92例行经皮椎体形成术治疗OVCF的患者的病历资料,根据术中使用骨水泥黏度不同分为对照组和观察组各46例。对照组使用低黏度骨水泥,观察组采用高黏度骨水泥。术后均随访6个月,比较2组患者术中情况及术前和术后3、6个月疼痛情况,伤椎功能情况和椎体形态;比较2组患者骨水泥渗漏情况。结果观察组手术时间显著低于对照组(P<0.05)。术后3、6个月2组患者Cobb角均降低(P<0.05),同一时间点组间比较差异无统计学意义(P>0.05);椎体前缘高度升高(P<0.05),同一时间点组间比较差异无统计学意义(P>0.05);视觉模拟评分(VAS)评分、Oswestry功能障碍指数(ODI)评分较术前均降低(P<0.05),同一时间点组间比较差异无统计学意义(P>0.05)。观察组骨水泥渗漏发生率(13.04%)显著低于对照组(30.43%)(P<0.05)。结论高黏度和低黏度骨水泥应用于经皮椎体形成术治疗OVCF均能显著改善患者疼痛症状及椎体形态和椎体功能,但使用高黏度骨水泥手术时间更短,同时可有效降低术后骨水泥渗漏发生率,改善预后,值得临床推广应用。Objective To analyze the effects of different viscosities of bone cement on vertebral body shape and bone cement leakage in patients with osteoporotic vertebral compression fracture(OVCF)during percutaneous vertebra formation.Methods Medical records of 92 patients with OVCF who underwent percutaneous vertebra formation surgery were retrospectively analyzed.According to the different concentrations of bone cement used during the surgery,they were divided into the control group and the observation group,with 46 patients in each group.The control group was operated with low-concentration bone cement,and the observation group was operated with high-concentration bone cement.The two groups were followed up for 6 months.Intraoperative conditions,preoperative and postoperative pain conditions at 3 and 6 months,injured vertebrae function and vertebral body morphology were compared between the two groups,and bone cement leakage conditions during follow-up were compared between the two groups.Results The operative time of the observation group was significantly lower than that of the control group(P<0.05).The visual analogue scale(VAS),Oswestry dysfunction index(ODI)and Cobb angle in the two groups were lower than those before operation 3 months and 6 months after operation(P<0.05).There was no significant difference in VAS score,ODI score and Cobb angle between groups 3 months and 6 months after operation(P>0.05).Three months after operation and six months after operation,the height of anterior vertebral body in the two groups was significantly higher than that before operation,and there was no significant difference between the two groups(P>0.05).The total incidence of leakage was 13.04%in the observation group during the follow-up,which was significantly lower than that in the control group(30.43%,P<0.05).Conclusion The application of high-concentration and low-concentration bone cement in percutaneous vertebroplasty for OVCF can significantly inhibit pain symptoms of patients,and can effectively improve vertebral body
