血清VEGF、CD34水平与颅脑损伤患者病情严重程度及预后的关系分析

摘　　要：目的研究颅脑损伤患者病情严重程度及预后与血清血管内皮生长因子(VEGF)、CD34水平间关系。方法回顾性分析2018年5月-2020年5月急诊收治的颅脑损伤患者82例,将其作为病例组,选择同期健康体检人群50名作为对照组。病例组患者均在治疗前进行血样采集,对照组于体检时采集血样标本,检查血清VEGF、CD34水平。观察病例组治疗前和对照组血清VEGF、CD34水平,统计病例组中轻型颅脑损伤、中型颅脑损伤、重型颅脑损伤、预后良好、预后不良例数,分别比较不同严重程度及不同预后患者血清VEGF、CD34水平,并对颅脑损伤患者病情严重程度及预后与血清VEGF、CD34水平相关性进行研究。结果病例组治疗前血清VEGF、CD34水平均明显高于对照组(P<0.05)。82例病例组患者中轻型颅脑损伤36例,中型颅脑损伤29例,重型颅脑损伤17例。轻型、中型及重型颅脑损伤患者预后不良例数依次增加,三组间比较差异有统计学意义(P<0.05)。轻型、中型及重型颅脑损伤患者血清VEGF、CD34水平依次递增(P<0.05);中型及重型颅脑损伤患者预后不良血清VEGF、CD34水平均高于预后良好(P<0.05)。Pearson相关性分析显示颅脑损伤严重程度与血清VEGF(r=0.717,P=0.000)、CD34水平(r=0.323,P=0.003)呈正相关;Pearson相关性分析显示颅脑损伤预后与血清VEGF(r=0.404,P=0.005)、CD34水平(r=0.366,P=0.012)呈正相关。结论颅脑损伤患者血清VEGF、CD34水平均较高,且血清VEGF、CD34水平上升与疾病严重程度和预后均呈正相关,对疾病早期诊断和预后预测有一定参考价值。Objective To study the relationship between disease severity,prognosis of patients with craniocerebral injury and serum vascular endothelial growth factor(VEGF),CD34 levels.Methods A retrospective analysis was performed on 82 patients with craniocerebral injury who were admitted to the hospital from May 2018 to May 2020,and they were enrolled as case group.50 healthy people who underwent physical examination in the hospital during the same period were enrolled as control group.The blood samples of all patients in the case group were collected before treatment,while blood samples in control group were collected when undergoing physical examination.The levels of serum VEGF and CD34 were examined.The levels of serum VEGF and CD34 in case group before treatment and control group were observed.The number of cases with mild craniocerebral injury,moderate craniocerebral injury,severe craniocerebral injury,good prognosis and poor prognosis in case group was statistically analyzed.The levels of serum VEGF and CD34 were compared in patients with different severity and different prognosis.The correlation between severity,prognosis of patients with craniocerebral injury and serum VEGF,CD34 levels was studied.Results Before treatment,levels of serum VEGF and CD34 in case group were significantly higher than those in control group(P<0.05).Among the 82 patients in case group,there were 36 cases with mild craniocerebral injury,29 cases with moderate craniocerebral injury,and 17 cases with severe craniocerebral injury.The number of cases with poor prognosis increased in turn in patients with mild,moderate and severe craniocerebral injury(P<0.05).The levels of serum VEGF and CD34 were increased in turn in patients with mild,moderate and severe craniocerebral injury(P<0.05).The levels of serum VEGF and CD34 in moderate and severe craniocerebral injury patients with poor prognosis were higher than those with good prognosis(P<0.05).Pearson correlation analysis showed that severity of craniocerebral injury was positively correlated wi
