摘 要:目的评价^(18)F-FDG PET/CT在心脏淋巴瘤的应用价值。方法回顾性分析2015年1月-2020年9月行^(18)F-FDG PET/CT全身显像的心脏淋巴瘤患者共34例,男22例,女12例,年龄(46±20)岁。患者手术病理结果或临床随访结果为依据分为原发性心脏淋巴瘤(PCL)组及继发性心脏淋巴瘤(SCL)组,分析心脏及心外病灶的位置、形态等影像学特征及病灶最大标准摄取值(maximum standardized uptake value,SUV_(max))的差异,探究^(18)F-FDG PET/CT对心脏淋巴瘤的临床应用价值。结果34例心脏淋巴瘤患者中,PCL 11例,SCL 23例,PCL组、SCL组淋巴瘤患者的心脏病灶SUV_(max)值分别为(12.44±8.16)、(6.50±4.60),差异有统计学意义(t=2.078,P=0.046);PCL组、SCL组淋巴瘤患者的心外病灶SUV_(max)值为(5.00±2.49)、(7.48±5.71),差异有统计学意义(t=-1.37,P=0.03)。且PCL的心脏病灶SUV_(max)值高于心外病灶SUV_(max)值,差异有统计学意义(t=1.986,P=0.041);SCL组的心脏病灶SUV_(max)值与心外病灶SUV_(max)值,差异无统计学意义(t=0.640,P=0.297)。结论^(18)F-FDG PET/CT全身显像对心脏淋巴瘤的诊断、分期、预后评估有一定的应用价值。Objective To evaluate the clinical value of ^(18)F-FDG PET/CT in cardiac lymphoma.Methods 34 patients,22 males and 12 females aged(46±20),were examined with ^(18)F-FDG PET/CT and diagnosed as cardiac lymphoma from January 2015 and September 2020.With surgical pathology or clinical follow-up results,patients were grouped to primary cardiac lymphoma(PCL)and secondary cardiac lymphoma(SCL)to analyze the differences in location,morphology and the maximum standardized uptake value(SUV_(max)),so as to explore the clinical application value of ^(18)F-FDG PET/CT for cardiac lymphoma.Results There were 11 PCL and 23 SCL patients.SUV_(max) of the heart lesions in the PCL and SCL groups were(12.44±8.16)and(6.50±4.60),respectively,with statistically significant differences(t=2.078,P=0.046).SUV_(max) of extracorporeal lesions in the PCL and SCL groups were(5.00±2.49)and(7.48±5.71),with statistically significant differences(t=-1.37,P=0.03).In addition,SUV_(max) of cardiac lesions in the PCL group was higher than that of extracardiac lesions,with a statistically significant difference(t=1.986,P=0.041).There was no significant difference between SUV_(max) value of cardiac lesions in the SCL group and SUV_(max) value of extracardiac lesions(t=0.640,P=0.297).Conclusion ^(18)F-FDG PET/CT has certain value in the diagnosis,stage and prognosis of cardiac lymphoma.
