心理干预在经皮脊柱内镜治疗腰椎间盘突出症的应用

摘　　要：目的对于腰椎间盘突出症合并抑郁或焦虑的患者,研究心理干预对其经皮脊柱内镜治疗的手术疗效的影响。方法回顾分析2017年1月-2019年12月收治的经皮脊柱内镜治疗腰椎间盘突出症合并抑郁及焦虑患者47例,其中24例为心理干预组、其余23例为对照组。在术前、术后第2天及术后第3个月随访完成数字评分法NRS疼痛评分,SF-36生理部分评分、抑郁自评量表及焦虑自评量表评估。结果两组术后第2天、术后第3个月随访NRS疼痛评分、SF-36生理部分评分较术前均有所改善(P<0.05);两组之间在对应随访时间点NRS疼痛评分、SF-36生理部分评分差异具有统计学意义(P<0.05)。结论对于腰椎间盘突出症合并抑郁或焦虑的患者,心理干预可显著提高经皮脊柱内镜治疗的手术效果。Objective To investigate the effect of psychological intervention on the surgical outcome of percutaneous spinal endoscopy for patients with lumbar disc herniation complicated with depression or anxiety.Methods 47 patients with lumbar disc herniation complicated with depression and anxiety treated by percutaneous spinal endoscopy from January 2017 to December 2019 were divided into two groups,of which 24 were in the psychological intervention group and the other 23 were in the control group.NRS pain score,SF-36 physiological part score,self-rating depression scale and self-rating anxiety scale were completed before operation,on the second day and the third month after operation.Results NRS pain score and SF-36 physical component summary were improved in the two groups on the second day and the third month after operation(P<0.05).NRS pain score and SF-36 physiological score were significantly different between the two groups at corresponding follow-up time points(P<0.05).Conclusion For patients with lumbar disc herniation complicated with depression or anxiety,psychological intervention can significantly improve the surgical results of percutaneous spinal endoscopy.
