摘 要:目的探讨抗苗勒管激素(AMH)水平与非优质囊胚移植临床结局的关系。方法回顾性分析冷冻胚胎冻融周期移植的240例患者,其中非优质单囊胚移植组129例,非优质双囊胚移植组111例,根据AMH水平分组,AMH<1.5 ng/ml为A组,AMH 1.5~4.5 ng/ml为B组,AMH>4.5 ng/ml为C组。分析3组AMH水平与非优质单囊胚移植、非优质双囊胚移植妊娠结局的相关性。结果在非优质单囊胚移植方案中,3组患者的临床妊娠率分别为30.30%、40.00%、52.17%,虽临床妊娠率随着AMH的升高有上升趋势,但差异无统计学意义(P>0.05);流产率分别30.00%、15.00%、25.00%,差异无统计学意义(P>0.05)。在非优质双囊胚移植方案中,3组患者的临床妊娠率分别为58.33%、50.00%、58.82%,流产率分别42.86%、37.50%、20.00%,妊娠率和流产率比较,差异均无统计学意义(均P>0.05),各组多胎率明显升高。双囊胚移植的临床妊娠率与单囊胚移植比较,差异有统计学意义(P<0.05),两组流产率比较,差异无统计学意义(P>0.05)。结论血清AMH水平在非优质囊胚移植中对妊娠结局的影响不大,AMH水平的高低并不能预估非优质囊胚移植的潜力,但非优质囊胚仍存在较大的移植价值。Objective To investigate the relationship between anti-Mullerian hormone(AMH)level and the clinical outcome of non-quality blastocyst transplantation.Methods A retrospective analysis was performed on 240 patients undergoing freeze-thaw cycle transfer of frozen embryos,including 129 cases in the non-high-quality single blastocyst transfer group and 111 cases in the non-high-quality double blastocyst transfer group.According to the AMH level,AMH<1.5 ng/ml was classified as group A,and AMH 1.5-4.5 ng/ml was classified as group B.AMH>4.5 ng/ml was group C.The correlation between AMH levels and pregnancy outcomes of non-high quality single blastocyst transplantation and non-high quality double blastocyst transplantation was analyzed.Results In the non-high quality single blastocyst transplantation regimen,the clinical pregnancy rates of the 3 groups were 30.30%,40.00% and 52.17%,respectively.Although the clinical pregnancy rates increased with the increase of AMH,the difference was not statistically significant(P>0.05).The abortion rates were 30.00%,15.00% and 25.00%,respectively,with no statistical significance(P>0.05).In the non-high quality double blastocyst transplantation program,the clinical pregnancy rates of the 3 groups were 58.33%,50.00%,58.82%,respectively,and the abortion rates were 42.86%,37.50%,20.00%,respectively.There was no significant difference in pregnancy rate and abortion rate(P>0.05),and the multiple pregnancy rate in each group was significantly increased.The clinical pregnancy rate of double blastocyst transplantation was significantly different from that of single blastocyst transplantation(P<0.05),but there was no significant difference in abortion rate between the two groups(P>0.05).Conclusion Serum AMH level has little effect on pregnancy outcome in non-high quality blastocyst transplantation,and the level of AMH does not predict the potential of non-high quality blastocyst transplantation.However,non-high quality blastocysts still have great transplantation value.
