摘 要:抗苗勒管激素(anti-Müllerian hormone,AMH)是转化生长因子-β(TGF-β)家族中的一员,目前血清AMH的测量已应用于临床妇科生殖内分泌的各个领域,现有研究表明,AMH是卵巢储备的可靠标志物,不仅可用于更年期的预测,而且在评估卵巢疾病手术或化学治疗后的卵巢损害方面也有一定价值。除此之外,在辅助生殖技术(assisted reproductive technology,ART)领域,AMH可有效预测受控卵巢对刺激的反应,在一定程度上为获得预后信息及个体化的卵巢刺激方案提供依据。近年来众多学者提出AMH是代替窦卵泡计数(antral follicle count,AFC)诊断多囊卵巢综合征(polycystic ovary syndrome,PCOS)的潜在工具,并发现血清AMH水平升高可能是PCOS主要病理生理学的一个促成因素。本综述主要探讨AMH与促黄体生成素(luteotropic hormone,LH)、胰岛素抵抗(insulin resistance,IR)、雄激素(androgen,A)的相关性,为PCOS的病理生理学研究及诊断标准提供新的思路。Anti-Müllerian hormone(AMH) is a member of TGF-β family. At present, the measurement of serum AMH has been applied in all fields of clinical gynecological reproductive endocrine. Current studies have shown that AMH is a reliable marker of ovarian reserve and can be used not only for the prediction of menopause, but also for the prediction of menopause. It is also valuable in assessing ovarian damage after surgery or chemotherapy for ovarian disease. In addition, in the field of assisted reproduction technology(assisted reproductive technology ART), AMH can effectively predict controlled ovarian response to stimulation, to a certain extent for prognostic information and individualized ovarian stimulation plan provides the basis. In recent years, many scholars have proposed that AMH is a potential tool to diagnose polycystic ovary syndrome(PCOS) instead of antral follicle count(AFC). It was found that the elevated serum AMH level may be a contributing factor to the main pathophysiology of PCOS. This review mainly discussed the correlation between AMH and luteotropic hormone(LH), insulin resistance(IR) and androgen(A), providing a new idea for the pathophysiology research and diagnostic criteria of PCOS.
