摘 要:目的本研究旨在描述双胎、双胎出生体重(BW)不协调、双胎出生顺序对新生儿结局的影响。方法收集2016年1月—2019年12月在本院新生儿重症监护病房(NICU)住院的双胎及单胎早产儿的临床数据资料,进行回顾性分析。对新生儿的特征、疾病和死亡率进行记录和比较。结果双胎中,新生儿发病率与胎龄(GA)呈负相关;双胎胎龄更小,出生体重更低,住院时间(LOS)更长,剖宫产率更高(P<0.05)。双胎中呼吸窘迫综合征(RDS)、心脏问题、胃肠道症状、贫血、低血糖的发病率更高(P<0.05)。中重度BW不协调与发病风险增加有关。出生顺序没有影响。结论早产是双胎中最常见的结局,也是发病率明显增加的原因。BW不协调可能在新生儿并发症的发生中起作用,需要进一步研究。Objective This study aims to describe the impact of twin birth, intertwin birth weight(BW) discordance and birth order on neonatal outcomes. Methods Clinical data of twin and single premature infants hospitalized in the NICU of our hospital from January 2016 to December 2019 were collected for retrospective analysis. Data on neonatal characteristics,morbidities, and mortality were collected and compared. Results The rates of observed neonatal morbidities in the whole population of twins were inversely related to GA. Twins were more likely to have lower GA and BW, longer LOS, and they had a higher risk to be born by cesarean delivery(P<0.05). Twins were more likely to have RDS, cardiac problems, gastrointestinal symptoms, anemia, hypoglycemia(P<0.05). Moderate and severe BW discordance were associated with increased risk for morbidities. Birth order had no effects. Conclusion Prematurity was the most common outcome in twins and accounted for the apparently increased risk in morbidities. BW discordance may play a role in developing neonatal complications and needs to be further investigated.
