摘  要:目的研究糖皮质激素对支气管哮喘患儿疗效与CHI3L1基因rs883125、rs4950928、rs10399805位点单核苷酸多态性之间的相关性。方法选取郑州大学附属儿童医院(河南省儿童医院,郑州儿童医院)2018年1月—2020年1月收治的支气管哮喘患儿158例,为观察组。对所有患者进行连续12周布地奈德吸入治疗。同期进行体检的健康儿童149例作为本次的参照研究对象,为参照组。采用TaqMan-PCR反应检测单核苷酸多态性(CHI3L1基因rs883125、rs4950928、rs10399805位点)。结果观察组总免疫球蛋白E(IgE)和甲壳质酶蛋白40(YKL-40)浓度分别为351.24(100.82,611.12)IU/mL、59.29(34.10,89.40)ng/mL,明显高于参照组(均P<0.05),且嗜酸性粒细胞比例在两组间差异无统计学意义(P>0.05)。观察组在rs883125位点的GG基因型频率高于参照组(P<0.05)。观察组在rs4950928位点CG基因型频率高于参照组,但在CC基因型中的频率则低于参照组,有统计学意义(P<0.05);该位点GG和CG基因型的哮喘组患儿总IgE和YKL-40水平显著高于CC基因型(P<0.05)。结论 CHI3L1基因rs4950928和rs883125位点基因多态性与哮喘易感性和糖皮质激素治疗效果明显有关。Objective To investigate the relationship between single nucleotide polymorphisms at RS10399805, RS4950928 and RS883125 of CHI3 L1 gene and glucocorticoid and therapeutic effect in children with bronchial asthma.Methods Totally 158 children with bronchial asthma admitted to our hospital from January 2018 to January 2020 were selected as the observation group. All patients were treated with budesonide inhalation for 12 weeks. A total of 149 healthy children who underwent physical examination in our hospital during the same period were selected as the reference subjects and named as the control group. SNPS of CHI3 L1 gene rs10399805, RS4950928 and RS883125 were detected by TAqman-PCR.Results The concentrations of total IgE and YKL-40 in the observation group were significantly higher than those in the control group(P<0.05), and there was no significant difference in the proportion of eosinophils between the two groups(P>0.05). The GG genotype frequency at rs883125 in observation group was higher than that in control group(P<0.05). The frequency of CC genotype at rs4950928 in the observation group was lower than that in the control group, but the frequency of CG genotype was higher than that in the control group(P<0.05). The levels of total IgE and YKL-40 in GG and CG genotypes were significantly higher than those in CC genotypes(P<0.05).Conclusion Rs4950928 and RS883125 polymorphisms of CHI3 L1 gene are associated with asthma susceptibility and glucocorticoid treatment.
