摘　　要：门静脉性肺动脉高压(POPH)是在门静脉高压基础上发生以肺动脉压升高、肺血管阻力增加而肺毛细血管楔压正常为特点的疾病。肝移植是POPH的有效治疗方法之一。目前我国尚缺少指导POPH肝移植围手术期管理的规范。中华医学会器官移植学分会围手术期管理学组、中国医师协会外科医师分会器官移植围手术期管理专业委员会基于国内外该领域循证医学证据,结合我国临床实践,制订《门静脉性肺动脉高压肝移植围手术期管理中国专家共识(2021版)》。该共识通过对POPH肝移植术前一般治疗、术前评估、术中管理以及术后并发症的预防与治疗进行规范,旨在提高POPH肝移植围手术期的诊断与治疗水平,改善预后。Portopulmonary hypertension(POPH)is a disease characterized by elevated pulmonary artery pressure,increased pulmonary vascular resistance,and normal pulmonary capillary wedge pressure on the basis of portal hypertension.Liver transplantation is one of the effective treatments for POPH.Currently,there is still a lack of guidelines for the perioperative period of POPH in China.Based on evidence-based medical evidences obtained from researches of this fields at home and abroad,and combined with clinical practice in China,the Perioperative Management Group of Chinese Society of Organ Transplantation from Chinese Medical Association and Organ Transplantation Perioperative Management Professional Committee of Chinese College of Surgeons from Chinese Medical Doctor Association formulate the Chinese expert consensus on perioperative management of liver transplantation in patients with POPH(2021 edition).This consensus forms standardized contents on the preoperative general treatment,preoperative evalua-tion,intraoperative management,prevention and treatment of postoperative complications of liver transplantation in patients with POPH,with the aims to enhance the diagnosis and treatment level during the perioperative period,improve the prognosis.
