某县级公立医院住院服务绩效分析

摘　　要：目的分析某县级公立医院住院服务绩效情况,为优化医院住院服务管理提供参考。方法运用DRG指标框架,纵向比较该医院2017—2019年的住院医疗服务能力、住院服务效率和住院医疗安全三个维度指标绩效,并与全省同级同类医院均值作比较。结果住院服务总产出(DRG总权重)逐年递增且均高于均值,病例组合指数(CMI)逐年下降且均低于均值,是典型的“规模数量型”模式;费用消耗指数和时间消耗指数每年均低于均值,服务效率属于高效型;低风险组死亡率和高风险组死亡率同呈逐年下降趋势。结论该医院需提高危重疑难病例占比以优化住院服务产出结构,促进时间消耗与费用消耗的良性协同控制,强化死亡病例的监控评估力度,进一步优化住院服务绩效。Objective To analyze the performance of inpatient service in a county-level public hospital,and to provide reference for optimizing hospital inpatient service management.Methods The DRG index framework was used to longitudinally compare the performance of the hospital's inpatient medical service capacity,inpatient service efficiency and inpatient medical safety from 2017 to 2019,and were further compared with the average of hospitals at the same level within the province.Results The total output of inpatient services(DRG total weight)increased year by year and was higher than the average value,and the case mix index(CMI)decreased year by year and was lower than the average value,which was a typical"scale and quantitative"model.Cost consumption index and time consumption index were lower than the average every year,and the service efficiency belonged to the high-efficiency type.The mortality rate of the low-risk group and the mortality rate of the high-risk group both showed a decreasing trend year by year.Conclusion The hospital needs to increase the proportion of critical and difficult cases to optimize the output structure of inpatient services,promote benign coordinated control of time consumption and cost consumption,strengthen the monitoring and evaluation of death cases,and further optimize inpatient service performance.
